Background: With the development of economy,the women pay more attention to the beauty of body,and the breast is critical key to form the beauty of body,which is why so many women want to have plump and firm breasts.The women who have congenital dysplastic breast and atrophic breast needs to seek for breast augmentation surgery to possess larger plump breasts.Therefore,the number of patients with breast augmentation surgery in recent years gradually increased and breast augmentation surgery becomes one of the top of plastic and aesthetic surgery.Now augmentation mammaplasty with silicone breast implants is still the main way of operation.However,many women can't accept prosthesis implanted into the breasts,and some patients are afraid of the high chance of capsular contracture after augmentation mammaplasty with mammary prostheses.After nearly 20 years of development,the technology of autologous fat transplantation gradually improved,especially in recent years,the proportion of patients who chose mammaplasty with autologous fat transplantation is gradually increasing.The cosmetic physicians and patients pay more and more attention to breast augmentation autologous fat transplantation.However,one of the main shortcomings of surgical procedure is that long-term adipocyte survival rate is low and need several operations to achieve the desired effect that the patients want to reach.To improve the breast fat postoperative survival rate,the problem of damage of adipose tissue during the process of suction and transplantation is urgent to be solved.Objective: This study improved the methods of traditional autologous fat transplantation way,such as taking fast and high pressure liposuction machine and a centrifuge separating mixed tissue to gaining and screening of adipose tissue.We used new methods as following to obtain a higher long-term adipocyte survival.The new methods are using preposed openings liposuction attract tube or syringes connecting syringes to pumping adipose tissue by manually low pressure,precasting lacuna measures before injection to minimize damage of micro strip funicular or micro mass sample of adipose tissue and increase the contact area of adipose tissue with the surrounding tissue.Methods: To study the data of 69 cases of small breast disease from three Plastic surgery hospital(Department of Plastic and Reconstructive Surgery of Southwest Hospital,Huamei plastic hospital,Dongjak Taiwan Plastic Surgery Hospital)ranging from March 2015 to January 2017 in Chongqing and randomly divided all the patients into two groups.36 patients chose traditional autogenous fat grafting for breast augmentation and 33 patients chose improved autogenous fat grafting for breast augmentation.All the patients were followed up for at least 6 months.We measured the preoperative breast volume and postoperative breast volume after 6 months in order to evaluate the difference of adipocyte survival rate between two groups.Meanwhile,postoperative complications,patient satisfaction survey of shape,size and quality of breasts of postoperative after 6 months were also evaluated.Biopsy of ostoperative breast tissue after fat grafting for 6 months and normal breast tissue specimens were taken and compared with breast tissue of PAAG breast augmentation.HE staining was used to analyze structure differences between autologous fat transplantation after breast adipose tissue and the surrounding breast tissue structure with normal breast tissue.CD68,CD3 antibodies immunofluorescence staining w ere performed to determine the presence of inflammation and immune dysfunction of breast tissue after fat grafting.Results: Adipocyte survival rates of patients with improved autogenous fat grafting is about 64.6%,which is significantly higher than 44.3% of those of patients with traditional autogenous fat grafting(p < 0.05).The improved group of patients with postoperative breast augmentation have higher satisfaction than the traditional group.Both two methods have few complications and no statistic difference(p > 0.05);HE staining showed that there was no structure differences between autologous fat transplantation after breast adipose tissue and the surrounding breast tissue structure with normal breast tissue.The immunofluorescence tests demonstrated that there was no difference of expression of CD68,CD3 between breast adipose tissue with autologous fat transplantation after 6 months and normal breast tissue(p>0.05),but expressions of CD68,CD3 of breast tissue from PAAG breast augmentation group were higher than normal breast tissue and breast adipose tissue after autologous fat transplantation after 6 months(p < 0.05).Conclusion:1,Adipocyte survival rate of autologous fat breast augmentation by improved method is higher than the traditional method.2,Both of improved method and traditional method have minimal complications3,Autologous fat transplantation had unnoticeable effect on the structure of the surrounding breast tissue.
