Investigate The Application Of Autologous Fat Granule Transplantation In Augmentation Rhinoplasty

Objective:In order to investigate the clinical application of autologous fat granuletransplantation in augmentation rhinoplasty.Methods:28 patientswith the diagnosis of low flat nasion and low nasal dorsum treated at 17th Department of Plastic Surgery (Plastic Surgery Hospital, Chinese Academy of Medical Science) were collected. Preoperative design was performed according to the patient’s requirement and thenasal feature.The fat granule was harvested by liposuction from waist abdomen or lateral thigh under intravenous anesthesia and local infiltration anesthesia. After purified, the fat was injected into the low flat nasionand nasal dorsum of 28 patients from multiple points and layers with even speed slowly.Then evaluate the operation result according to the change of nasofrontal angle,the rising height of nasion and nasal dorsum,complicationsand satisfaction measurement.Results:All 28 patients were followed up for 3 to 24 months,21 cases achieved the expected effect by injecting once and 7 cases achieved satisfactory results after reoperation, the amount of autologous fat is 1.5ml to 4.0ml. Nasofrontal angle change is from-12°to 12°.Theheightof nasion and nasal dorsum were higher, having a more natural and harmonious junction with the apex of nose. No severe complications.All patients were satisfied with the operation result.Conclusion:Autologous fat grafting injection for augmentation rhinoplasty has advantages of no rejection reaction, natural permanent effect, minimal injury and mild complications. The key point lies in the rising height of nasion and nasal dorsum rather than the change of nasofrontal angle. This makes the junction with apex of nosemore natural and harmonious. In addition,nasion and nasal dorsumwith higher heightcould extend the length of the nose and narrow the size of the apex relatively. Thismethod applies to low flat nasion, low nasal dorsum, as well as mild hump nose and saddle nose.It does not apply for the patient who has a high requriment about the apex and columella of the nose, severehumpnose, saddle nose,as well as the patient refusing once more fat injection.Autologous fat has a high absorbtion after transplantation in augmentation rhinoplasty. Thus, once more injection could achieve better apperance.
