Significance and objectivesIn recent years,the domestic medical environment is becoming grim-the events of doctor attacked by the patient were reported frequently.According to Zhao Min et al in 2017 published in the "medicine and philosophy",from 2000 to 2015,Chinese media reported a total of 290 cases of violence injury medical events.It shows an upward trend-the cases of violence occurred in 2013 are 4.5 times than 2012;from 2013 to 2015,there are 141 cases reported,accounting for 48.3%of the total.The situation is more and more serious.It is now a social problem to be solved on protecting the safety of medical personnel and preventing the violence injury medial events from happening.Otherwise,there would be a serious impact on hospital operations.There are few studies on the aggressive behaviors of patients in China.However,the existing research is limited to psychosis patients,and lack of overall patient samples.In domestic and foreign research on aggressive behavior of patients,it is mainly on quantitative research while qualitative research is most likely ignored.Therefore,in order to dig out the deep factors of the aggressive behaviors of the patients and extract the "local characteristics" under Chinese medical background,this study intends to use the quantitative research and qualitative interview method to study the related factors of the aggressive behavior of the patients.The research is trying to provide a new path for the intervention of the reality of doctors and patients.Study 1:Study on current status and influencing factors of aggressive behavior of patientsResearch objects and methods1.Sampling methods and research objectsIn this study,benefited from the convenience of medical institution,seven hospitals are picked up from the affiliated hospitals at all levels.Using anonymous survey,a total of 370 questionnaires were randomly issued to hospital patients.After removing invalid ones,338 answers were received,i.e with effective rate of 91.4%,including 161 male patients,accounting for 47.6%and 175 female patients,accounting for 51.8%.The age range of the patients is(42.8 ± 18.88)years.2.Evaluation tools(1)Self-compiled general situation questionnaire,including gender,age,nationality,education level,marital status,time of illness,satisfaction with doctors,family economic status,attitudes and cognition of doctor-patient relationship.(2)Evaluation tools affecting the factors associated with aggressive behavior of patients,including the Chinese version of the Buss-Perry attack scale,the Wickers Physician Trust Scale(WFPTS),and the Social Support Scale.The results1.Chinese patients are in the middle of the offensive level,patient aggression total is(54.08±13.55),and the main features of the performance are high volatility and low physical attack.2.The more satisfied with the higher medical services,the lower the level of attack.The smaller the effect of treatment on the patient's economy,the lower the patient's aggressive level.Patient's aggressive linearregression results display(F=8.729,p=0.000,R2=0.172)3.Comprehension of social support and medical/patient trust is the patient's protective factors.Improving the patient's comprehension of social support and the establishment of a good doctoral trust helps to ease the patient attack.Comprehension of social support and medical/patient trust related results are display(r = 0.39?0.41?0.31).Study 2:A qualitative study of the relevant factors of aggressive behavior of patientsThe research objects and methods1.Sampling methods and research objectsEnroll volunteers from the eligible patients by using purposeful sampling method.The study objects are patients taking attack behaviors after encountering unpleasant experience during the medical treatment process.After the respondents were selected,they were introduced the relevant issues of the study,including the principles of voluntary and confidentiality,as well as the purpose and content of the study,and reading of the informed consent form.In the case of mutual recognition,informed consent form was signed.There are a total of 35 interviewees,while the sample age stratification is,18 people in 20-35 years old,14 people in 36-50 years old and 3 people in more than 50 years old.2.Research methodsUsed key event interview method and generic analysis method,rooting theory research and analysis.The results1.Anger is the most common mood among patients while there are conflicts between doctors and patients,and speech attacks and devastating attacks are the most common forms of patient attacks.2.The negative experience in the treatment process is the main factor that triggers the patient's aggressive behavior.3.Patient's cognitive bias on relationship between doctors and patients and the state of the patient's aggressive behavior plays an important role.4.Frustration on medical experience and below expectation treatment results caused questions and mistrust while the patients trusted the doctors originally.5.Increasing medical costs increases the economic burden on patients,making the relationship between doctors and patients further tense.6.The imperfect health system and unreasonable allocation of resources further aggravate the aggressive behavior of patients.Main conclusions1.Chinese aggressive characteristics are with high impulse and low physical attack on the patient's performance,the behaviors of patients with aggressive behavior are usually speech attacks and destructive attacks.2.Anger is the most common mood that a patient produces when a doctor-patient conflict occurs,the negative emotional experience during the treatment process will awaken the patient's aggressive behavior.3.The greater impact of treatment on the patient's economy,lower the patient's satisfaction with medical services,comprehension of social support and lack of trust between doctors and patients is a risk factor for patient attacks.The characteristics and innovation of this study1.Characteristics in research methods.Based on the traditional quantitative research,this paper combines the qualitative research method and makes use of the key event interview method to make up the deficiency of the traditional quantitative research which cannot know the root cause from the surface feature.It further studies the patient's attack and its influencing factors.2.Research content development.This study extends the sample to the general patient population in the hospital environment and further explores the influencing factors of aggressive behavior in this particular group of patients.The shortcomings and limitations of this study1.Limitations of research methods.Key event technology is widely used in exploratory research,but as a qualitative technique,although the author strictly followed its analysis process,it is difficult to achieve 100%objective.Therefore,the researcher should strive to objectively reproduce the story itself,as far as possible not mixed with my own judgments.2.Sample limitations.As all the samples in this study are from Guangdong Province,it should pay attention to the characteristics of the region when the results were extended to other cities and rural areas.
