Construction Of Community Health Management Model Based On PRECEDE-PROCEED Model

With the continuous improvement of the public health service system,the support of national policy,high-tech means,the way of life,disease spectrum,death spectrum changes,people are more and more requirement on health.And since 2000 years,China has entered the aging society and the high burden of chronic disease at the same time,chronic disease has become the main public health problems in China,with morbidity and high medical costs,long duration of disease,characteristics of services ",prevention and treatment of chronic diseases and delay.The large population of our country,regional economic development is uneven,the gap is large,the per capita resources shortage.We can not copy the foreign Health management mode,the only way is learn from the successful experience of other countries,combined with their own conditions,health management mode of development with Chinese characteristics.In this context,how to effectively develop health management,reduce the incidence of chronic diseases,improve the health level of people is particularly important.ObjectiveThis study is based on the project of Science and Technology project of Guangdong province "Construction and promotion of chronic disease community health management experimentation area based on PRECEDE-PROCEED Model",through understanding of community health management and PRECEDE PROCEED Model theory,according to the city residents of our community health needs,combined with China's actual situation and compare with the health management experience and the mode of foreign,combined with the modern network information technology.Based on the community health service center,this primary medical platform,to explore the personalized service health management model that applicable to the health management of urban residents,In order to meet the needs of city community residents to provide safe,effective,convenient,fast,high quality,low price,continuous,comprehensive health management services,improve their health,reduce medical expenses,provide reference for the perfection of China's medical and health system.MethodThe theory research adopts the literature research method,mainly from the domestic and foreign database and related websites.Read the basic theory of related literature on health management and PRECEDE-PROCEED Model,At the same time,explore and in-depth analysis the health management mode for the literature,to understand the current situation and existing problems,and lays a theoretical foundation for the personalized service model applicable to urban community residents.The survey data come from the on-the-spot investigation and network official data of Guangzhou city in Guangdong province,field investigation adopts typical sampling method,Survey objects come from the Residents from 9 community health service centers in Tianhe District,Guangzhou.The subjects of the study group were referred to the relevant literature and scales,and the self-made questionnaire was used to conduct a one to one questionnaire survey.The questionnaire includes three parts:general demographic characteristics,self health management,willingness and needs of community health management.Statistical method uses EiPData3.1 software to double track data,and SPSS20.0 statistical software for data analysis,Chi square test was used to analyze the differences in self health management among different demographic factors,Two categories of logistic regression were used to analyze the influencing factors of self health management,with P<0.05,the difference was statistically significant.ResultThe fifth national health service survey showed China's urban and rural chronic disease prevalence rate was 36.7%and 29.5%.Compared with the fourth health service survey in 2008,the result was increased by 8.5%.The proportion of self health management of the respondents was 72.9%,Single factor analysis showed that those residents of male,high age,high education level,fixed occupation,A large family population,high family income,suffering from chronic diseases showed a higher proportion of self health management(P<0.05).Two categories logistic regression analysis showed male,the higher educational level,the more family population,the better health,the healthy lifestyle,the purchase of insurance,the willingness to invest in health and Follow your doctor's advice are the contributing factors for the residents'self health management.Single factor analysis showed the people who were that young,highly educated,more sickness in a year,without chronic disease,middle income,more monthly health investment,self-rated health is better,participate in community health education activities,and with self health management showed a higher Willingness to participate in community health management(P<0.05);The most important service items for residents are physical examination(66.1%),health education(53%),diet management(53%)and chronic disease management(51.4%);Residents are most likely to use mobile phones(53%)to get health education knowledge.All the above differences were statistically significant.Conclusion1.Guangzhou city residents showed a high Willingness to health management,community health management should be vigorously implemented,establish a health management model covering the whole population,expanding the content and channel of health education is an important way to improve the health literacy of residents,effective prevention and control of chronic diseases and reduce the burden of disease.2.The self health management of Guangzhou residents is in good condition,In the community health management we shoud paid attention to the improvement of self health management ability among women,young people,low education,low income and sub healthy people,broaden the way and content of health education,to carry out intervention measures to the influence factors of self health management.3.Guangzhou city residents showed a higher health education needs.In the development of community health education,we should keep pace with the times,and health needs of residents as the guide,create a community health education knowledge sharing platform,using modern Internet media and community resources,to realization information management of health education4.Based on the survey results of health management demand of Guangzhou residents,construction a health management model based of PRECEDE PROCEED Model.Community health management plays an irreplaceable role in community health services,effective management can serve the residents' health,block the development of chronic diseases,and improve the quality of life of residents.Community health management should be based on the community,with health promotion and behavioral risk factors intervention as an important means and content.Apply the PRECEDE PROCEED Model to community health management,from the 9 stages and in-depth analysis and intervention,of the health of residents,can provide more scientific and systematic of the health management to residents.This is not only in line with national construction of "healthy China"",The implementation of grading treatment system,requirements for the implementation of a comprehensive strategy for the prevention and treatment of chronic diseases,It is the most economical and effective method to benefit people at present.
