Objective 1.To observe the clinical effect of the treatment of giant hepatic cysts by ultrasound-guided sclerotherapy combined with tube drainage,and summarize the clinical experience,in order to promote the clinical work better.2.To discuss the influence factors on the treatment outcome of the treatment of huge hepatic cysts by ultrasound-guided catheter sclerotherapy,and analyze the high risk factors of the recurrence of huge hepatic cysts by ultrasound-guided sclerotherapy combined with tube drainage,then improve better guideline of clinical therapy.Methods A total of 72 patients who were chosen from January 2010 to January 2016 in the first people's Hospital of Jingzhou with huge hepatic cysts(larger than 10 cm in diameter)was treated by ultrasound-guided sclerotherapy combined with tube drainage first.All patients were analyzed retrospectively to seek causes of recurrence and use the single factor analysis to research into the relationship between outcome of the treatment of huge hepatic cysts by ultrasound-guided catheter sclerotherapy and factors,such as age,gender,volume of the cyst,the maximum line of the cyst,the number of cysts,separation of cyst cavity after hardening and patients with chronic liver injury.Then cyst recurrence variables would be used into the regression statistics to establish Logistic method regression model.Results 1.All cases,for once,were managed by puncture and catheterization,and were completed the follow-up without loss.72 cases of huge hepatic cysts were effective in 58 cases and recurrence in 14 cases.The effective rate was 80.6% and recurrence rate was about 19.4%.Among 72 cases of huge hepatic cysts,there were 19 cases with mild abdominal pain and right shoulder pain,and there were 8 cases with drunk like mild reaction,such as dizzinessa and vomiting.After the patients lied down for rest,the symptoms were relieved effectivly without any other special treatment.There were no serious complications during and after the sclerotherapy,such as ethyl alcohol allergies,sclerosing agent leakage,severe infection and obvious hemorrhage.2.Univariate analysis showed that gender,the number of cysts and patients with chronic liver injury had no significant correlation with recurrence of giant hepatic cysts,while the age of patients,separation of cyst cavity after hardening,volume and the maximum line of the cyst were significantly related to final cure outcome.Multivariate analysis showed that separation of cyst cavity after hardening,volume and the maximum line of the cyst were the risk factors for the recurrence of cyst.Conclusion 1.The sclerotherapy with puncture drainage of huge hepatic cysts is safe,effective and worthy of clinical application by ultrasound-guidance.2.For patients with huge liver cysts,treatment should be individualized,and choose the appropriate treatment according to the specific circumstances.The larger the volume and the maximum line of the cyst is,the higher the risk is by this way.When volume and the maximum line of the cyst is large and separation of cyst cavity after hardening,the sclerotherapy should be operated once more the next day.
