Objective:The purpose of this study was to find and evaluate a conservative treatment of large mandibular odontogenic cysts, which could maintain the continuity of the mandible and conserve adjacent vital structures of the cyst, as well as correct maxillofacial malformation and less interfere the growth of the mandible.Methods:9 patients who had large mandibular cysts were collected in the oral and maxillofacial surgery department of the second afflicted hospital of Zhejiang University from 2000. 1. 1 to 2003. 6. 30. These cases, 3 females and 6 males, aged between 8 and 22, treated by 1)surgical enucleation or curettage 2)tooth extraction and 3)filled with iodoform gauze under general or local anesthesia after examination. Those cases suspected as odontogenic keratocysts were treated by phenol Cauterization .additionally. Postoperative panoramic radiograph and clinical examination were performed on all cases monthly. Bone regeneration and reduction of the residual cavities were evaluated with a computed analysis of preoperative and postoperative panoramic radiographs.Results:2 or 3 months after treatment maxillofacial malformation of all cases were corrected. The computed analysis of the postoperative panoramic radiographs showed mean values of reduction in area of the residual cavities of 27. 2%, 63. 8% and 90.8% after 1. 3 and 6 months, respectively. The thickness of regenerative bone of the mandibular or ramus added 4. 5> 16. 3 and 27. 2mm after 1. 3 and 6 months, respectively. The density of regenerative bone was approaching to normal bone after 12 months. Conclusion: This method of treatment of the large mandibular cyst could maintain the continuity of the mandible , conserve adjacent vitalstructures of the cyst, correct maxillofacial malformation soon and less interfere the growth of the mandible and so on.This method is applied to adolescent especially.
