Study On The Clinical Intervention Of Danzhijiangtang Capsule With Early Diabetic Lower Extremity Vascular Disease And The Influence Of Ang1,Ang2 And Tie2 Protein Expression In Rats

Objective:To observe the effect of Danzhijiangtang Capsule on type 2 diabetic patients with early lower extremity vascular disease intervention and the influence on Serum levels of Ang-1,Ang-2and Tie-2 on the expression of Ang-1,Ang-2 and Tie-2 and from the molecular and genetic level to research the effect of Danzhijiangtang Capsule on the expression of Ang-1,Ang-2 and Tie-2 protein in type 2 diabetic lower extremity vascular disease model rats.To explore the mechanism of Danzhijiangtang Capsule on patients with early lower extremity vascular disease and the role of angiogenesis in diabetic lower extremity vascular disease model rats with endothelial injury.Methods:Clinical Research: 98 T2 DM patients with lower extremity vascular disease were randomly divided into control group 49 cases and treatment group 49 cases.Two groups of patients were given the same basic treatment,the control group was treated with cilostazol,and the treatment group were given Danzhijiangtang Capsule based on the control group therapy.4 weeks as a course of treatment for a total of 8 courses.To observe the two groups of patients before and after treatment of TCM syndrome integral,detect the levels of blood glucose,blood lipids,Ang-1,Ang-2,Tie-2 and Lower extremity arterial diameter,IMT,ABI,transcutaneous oxygen partial pressure.To investigate the effect of Danzhijiangtang Capsule on diabetic patients with early lower extremity vascular disease clinical intervention.Experiment:70 male healthy SD rats were fed for a period of 1 week,and randomly divided into 2groups:blank group 10 and experiment group 60.The blank group were fed with ordinary diet and experimental group were treated with streptozotocin combined with high fat diet to establish diabetes model.The model was established successfully in 49 rats were randomly divided into the following 5 groups:western medicine group 10 rats,model group 9 rats,high dose group 10 rats,middle dose group 10 rats and low dose group 10 rats.The blank group and model group were given normal saline,western medicine group were given intragastric administration of cilostazol,high,medium and low dose group were given different dose of Danzhijiangtang Capsule.Each group was intragastric for 12 weeks.Rats in each group were observed the changes of behavior and weight,detected the levels of FPG,blood lipids,Ang-1,Ang-2,Tie-2 and the expressions of Ang-1,Ang-2,Tie-2 Protein expression level in Gastrocnemius muscle.Results:1.Clinical Research: Clinical observation showed that the total effective rate was 87.23% in the treatment group and 66.67% in the control group after 32 weeks of treatment.TCM integral Syndrome of the treatment group was better than the control group(P<0.01).After treatment,the FPG,2h PG and Hb A1 c of the two groups were lower than those before treatment,and compared with the control group,the treatment group decreased significantly(P<0.05).After treatment,TG,TC,HDL,LDL of the two groups were improved(P<0.05 or P<0.01),and compared with the control group,the treatment group improved significantly(P<0.05).Compared with before treatment,the two groups of patients after treatment of lower extremity arterial blood vessel diameter increased,femoral artery intima-media thickness,ankle brachial index and transcutaneous oxygen partial pressure increased significantly(P<0.01),and the treatment group was better than the control group(P<0.05).The serum Ang-1 and Tie-2 levels in the treatment group and the control group were significantly higher than those before treatment,Ang-2 level decreased(P<0.05),and the treatment group was significantly better than the control group(P<0.01).2.Experiment: Animal experimental results showed that compared with the model group,FPG,Hb A1 c decreased in the high,middle and low dose groups and Western medicine group(P<0.01),and that decreased significantly in the high,middle and low dose groups.Compared with model group and western medicine group,TG,TC,HDL and LDL were significantly improved in the high,middle and low dose groups(P<0.05 or P<0.01).Compared with the blank group,the levels of Ang-1 and Tie-2 in the model group were significantly lower and the level of Ang-2 increased(P<0.01).Compared with the model group,the levels of Ang-1 and Tie-2 in western medicine group and high,middle and low dose groups were increased,and the level of Ang-2 was decreased,and the level of Ang-2 was decreased.Compared with the blank group,the protein expression level of Ang-1 and Tie-2 in the model group was significantly lower(P<0.01).Compared with the model group,the protein expression levels of Ang-1 and Tie-2 in western medicine group and high,middle and low dose groups were increased(P<0.01),and high and middle dose groups were better than western medicine group(P<0.05).Conclusion:Danzhijiangtang Capsule can effectively improve the early type 2 diabetic patients with lower extremity vascular disease of lower limb ischemia Symptom and Qi deficiency and blood stasis syndrome.Its mechanism may be related to lowering blood glucose,glycosylated hemoglobin levels,regulating blood lipids,increasing the expression level of Ang-1 and Tie-2 improving endothelial state,promoting angiogenesis,increasing blood perfusion.
