The Effect Of DJC On Early DKD Patients And The Effect Of SDF-1? And P27 In Serum Of Diabetic Rats

Objective: To observe the clinical effect of DJC on the early DKD patients and the intervention of SDF-1? and PEDF with the effect of replenishing qi,nourishing yin and activating blood circulation.To observe the effect of SDF-1? and P27 in serum of DJC diabetic rats.Objective to analyze the correlation between inflammatory markers and DKD,and to explore the influence of DJC on DKD and related inflammatory markers,and to explore the target and mechanism of DJC in preventing and treating diabetic nephropathy.Methods:1.clinical research:90 cases of early DKD were randomly divided into DJC group and control group,45 cases in each group.The control group was treated with simple western medicine,while the DJC group was treated with western medicine and DJC for 12 weeks.The TCM syndrome,FPG,2h PG,Hb A1 c,TC,TG,LDL,HDL,m Alb,ACR,Cys C are measured before and after treatment.All patients were examined with Doppler ultrasound before and after treatment.At last,the levels of SDF-1?and PEDF were measured by enzyme linked immunosorbent assay method..2.Animal experiment:48 rats were divided into normal control group,model group,high-and low-dose DJC groups,pioglitazone group.Except the normal control group,the rest of the animal given high fat diet for 4 weeks,according to 35mg/Kg-1 rats by intraperitoneal injection of streptozotocin solution.The rats were administrated for 8 weeks.The serum levels of SDF-1? and P27 were determined by double enzyme linked immunosorbent assay,and blood glucose and glycosylated hemoglobin were measured.Results:1.Clinical Test:The clinical research showed that the total efficiency of DJC group had proved much better(86.1%)than of the control group(59.1%)(P<0.05).After treatment,FPG,2h PG,Hb A1 c,TC,TG,LDL,HDL,m Alb,ACR,Vmax,RI,Cys C,SDF-1? and PEDF levels in two groups of patients were improved compared with before treatment(P<0.05,P<0.01).After treatment,HDL and SDF-1? in group DJC were higher than those in control group.On another hand,after treatment,the indexes of renal function in DJC group were better than those in control group.After treatment,RI,PEDF and Cys C in the DJC group were lower than those in the control group(P<0.05,P<0.01).2.Experiments:The results of animal experiment showed that compared with the normal control group,the blood glucose,Hb A1 c,TG,SDF-1? and P27 were significantly different in the model group(P < 0.01).After 8 weeks of treatment,the blood glucose,Hb A1 c,TG,P27 levels of pioglitazone group and DJC high and low dose group were significantly lower than that of model group,and the levels of SDF-1? were significantly increased(P<0.01).There was no significant difference between pioglitazone group and DJC group(P>0.05).Correlation analysis showed that the blood glucose was significantly correlated with SDF-1? and P27(P<0.01).Conclusions:DJC could effectively improve the TCM syndrome,renal function and related inflammatory indexes in early DKD patients.And it could increase the level of serum SDF-1?in diabetic rats,decrease the level of P27,regulate glucose and lipid metabolism,delay DKD,and protect renal function.
