A Study On The Service Ability Of Pre-hospital Emergency In Urumqi

Objective:To investigate the staff and the resources of pre-hospital emergency and epidemiological characteristics of pre-hospital emergency patients in Urumqi,to study the current situation and the utilization of first aid resources in Urumqi,in order to found that the lack of pre-hospital emergency services,strengthen the pre-hospital emergency service system construction,rational allocation of pre-hospital emergency resources,improve the ability of emergency rescue capacity and put forward rationalization proposals in Urumqi.Methods:Use questionnaire survey and face-to-face interviews to investigate the allocation of first aid resources of the Prehospital first aid network hospitals and sites from January 1,2014 to December 31,2016: including human resources(work situation,job satisfaction)And Material equipment(emergency vehicles and emergency equipment configuration status,etc.),To understand the resources status quo of pre-hospital emergency network hospitals and sites;Combined with Urumqi City 120 emergency center daily dispatch command data,retrospective analysis 160 117 cases of pre-hospital emergency patients from January 1,2014-December 31,2016.Use ICD-10 to classify all cases.Analysis the crowd,time,regional distribution of pre-hospital first aid patients.Understand the service ability of 120 and find out the factors that affect the use of resources services.Results:1.There are 26 emergency substation in Urumqi,there are 4 directly and 4 pre-hospitals have full-time doctors and nurses,another 18 do not have full-time pre-hospital emergency medical staff.2.Prehospital first aid resource allocation:(1)Material equipment: there are pre-hospital 32 emergency vehicles;(2)Human resources: There are 260 staffs in 2016,include 97 males and 163 females,the average age: 32.20 ± 8.321;The age distribution is mainly under the age of 30(51.2%);the academic distribution is mainly undergraduate;the job distribution is mainly primary(73.85%);the number of years of work is mainly 3 to 10 years(48.35%),followed by the year? 3(38.08%).(3)There were statistical differences between the job satisfaction and the gender,age,education,working life of the medical staff in 2014,the situation of the job,the job title,the monthly income,the learning opportunity and the future development space in 2014(P<0.05);Undergraduate,monthly income of 2000 ~ 3000 ~ more dissatisfied with the working state(OR<1,P<0.05);(4)There were statistical differences between the job satisfaction and the age,education,in the situation,title,monthly income,the future development of space,whether to enjoy the normal leave,whether the uniform with uniform in 2016(P<0.05);The older,the higher of education are less satisfied with the work;(5)Comparison of job satisfaction in different years: 2014 annual dissatisfaction accounted for 37.9%,2016 accounted for 13.85%.3.Disease spectrum analysis(1)The peak month is December,May,July and 9-10 months;peak time is 22: 00-24: 00,10: 00-12: 00,14: 00-16: 00.(2)The admissions area is mainly in the new urban area and the Shayibak area(3)The top six categories of disease classification are the injury,cardiovascular and cerebrovascular diseases,poisoning,respiratory diseases,neurological diseases,digestive diseases;(4)Age distribution: the mainly age is 60 and elderly;Conclusion:1.The construction of emergency network system is irrational in Urumqi,a full-time pre-hospital team need to be equipped,a reasonable layout of the first aid site need to be set;2.Strengthen the hospital emergency personnel training,increase financial investment,improve pre-hospital emergency medical staff income level;To develop a separate job title promotion system to improve the job satisfaction of first aid personnel;3.The configuration of ambulances and onboard equipment should be continually updated and improved in conjunction with actual needs and utilization;4.Based on the analysis of disease spectrum and epidemiological characteristics,strengthen the professional and technical training for the city 's multiple diseases,improve the treatment capacity,rational allocation of personnel and vehicles,optimize the allocation of resources,improve service capacity.
