Efficacy Of OBS Treating Periprosthetic Femoral Fractures Of Vancouver B1 And C

Objective: We use Ortho-Bridge System to treat Periprosthetic Femoral Fractures of Vancouver B1& C,observeing the bleeding of intraoperation,operation time,fracture healing,limb shortening,Harris hip score and other indicators to evaluate its efficacy,to provide clinical basis for treating PFF of personalized OBS,provide a new way for treating PFF.Methods: 23 cases of periprosthetic femoral fracture after hip arthroplasty in the Department of orthopedics,Yanan Hospital Affiliated to Kunming Medical University met the inclusion criteria were collected in 2012.01-2015.12,using OBS to treat and with complete follow-up and related information.There were 12 males and 11 females;the maximum age 89,the minimum age 67,the average age 78.5;14 cases on the left side,9 cases on right side;19 cases after total hip arthroplasty,4 cases after hemiarthroplasty;9 cases of bone cement prosthesis,14 cases of biological prosthesis.The operation time was 2-7 days after injury.In this study we use Vancouver classification,B1 type 15 cases,C type 8 cases,1 B1 type merge femoral supracondylar fracture.Preoperative three-dimensional reconstruction of CT or print out the femoral model(1:1)with 3D-printer,group OBS individually for fastening fracture in operation.Defending limb deep vein thrombosis,pulmonary infection or other complications postoperative.Observe intraoperative bleeding,operation time,limb shortening,the incidence of complications;Follow-up in 3?6?12?18?24 and 36 months after the operation,evaluate the fracture healing time with X ray;Harris score of bilateral were statistically analyzed;preoperative and postoperative lower limb shortening length were statistically analyzed.Through observe and analyse the above indicators,evaluatiing the clinical efficacy of the OBS treating PFF synthetically.Results: Patients received at least 12 months follow-up,the average healing time were 4.2 months,after operation 12 months Harris score was 87.04±2.82,the excellent rate was 87%;The affected side of Harris score after operation 6 months was significantly different compared with preoperative,the difference was statistically significant,The affected side of Harris score after surgery6 and12 months was no significant difference,but the difference was statistically significant;compared the length of limb shortening in preoperative and postoperative,the difference was statistically significant.OBS has a significant effect on the treating of PFF,can effectively restore the patient's daily life.Conclusion: OBS of the "customization" to achieve "three-dimensional fixed",treating PFF of Vancouver B1,C type,can restore the limb length and hip joint function effectively.short and medium term's clinical curative effect was satisfied.OBS is a new and effective internal fixation system on the treatment of PFF.
