Modify Stoppa Approach With Iliac Fossa Approach In Treatment And Analysis Of Type C Pelvic Fractures

PURPOSE We present our experience of using a modified Stoppa approach combined with iliac fossa approach to type C pelvic fractures in patients in reference to fracture reduction and fixation,technical aspects,and the incidence of complications.Sum up experience,discuss and further improve the treatment effect,in order to promote and to get a wider range of clinical use etc.METHODS Form Sep.2014 to Jul.2016,17 cases of type C pelvic fractures were treated by Modify Stoppa approach with iliac fossa approach.We have a retrospective analysis.The study group included 12 male and 5 female patients with an average age of42.88 years(range,11–70 years),All the patients were followed average 1 years(6-22months).Fracture reduction and postoperative hip functions were assessed by Matta criterion and Majeed scoring system respectively.Operation time,blood loss,blood transfusion,injury of sciatic nerve and complications such as infection were recorded.RESULTS These cases of type C pelvic fractures treated by Modify Stoppa approach with iliac fossa approach were followed average 1 year.Average blood loss in these operations was 750ml(450-1400ml).Average blood transfusion was 4.2U(2-8U).Average Operation time was 210 min(120-500min).Pelvic fractures are osseous healing,fracture healing 2-6 months,mean 3.5 months.Postoperative complications occurred in 2 cases,both for deep vein thrombosis of lower limbs.According to Matta criterion,It showed excellent in 12 cases,good in 4 cases,fair in 1 case andpoor in 0 case.The excellent rate is 94.1%.At the last follow up,Majeed scoring showed excellent in 10 cases,good in 4 cases,fair in 1 case and poor in 2 cases.The excellent rate is 82.4%.CONCLUSION These cases of type C pelvic fractures thought Modify Stoppa approach with iliac fossa approach were absolutely followed average 1 year.According to Matta criterion,It showed excellent in 12 cases,good in 4 cases,fair in 1 cases and poor in0 cases.The excellent rate is 94.1%.At the last follow up,Majeed scoring showed excellent in 10 cases,good in 4 cases,fair in 1 case and poor in 2 cases.The excellent rate is 82.4%.Average blood loss in these operations was 750 ml.Average blood transfusion was 4.2U.Average Operation time was 210 min.Pelvic fractures are osseous healing in 2-6 months,mean 3.5 months.Postoperative complications occurred in 2 cases,both for deep vein thrombosis of lower limbs.Other patients had no infection and no important nerve vascular injury except 1 case.Type C pelvic fractures were treated by Modify Stoppa approach with iliac fossa approach.Surgical field shows good,operation is relatively simple.It reduces the operation difficulty and improves the high reduction rate of pelvic fracture.It well solves the ilium fractures and sacroiliac joint dislocation.Fracture's degree of satisfaction and lower limb function have recovered well,forward hip function well.The fracture surgery is satisfactory and two lower limb functions recover well.There are fewer complications.
