Objective:High energy injury can cause the anterior pelvic ring fractures,there are many important structures around the pelvic ring,the displacement blocks caused by the injury can cause peripheral vascular,nerve and visceral injury,the disability and mortality rate is higher in the department of orthopedics trauma diseases,the survival and quality of life of patients are affected seriously.Pelvic fracture is a common disease in clinical department of orthopedics,with the increasing rate of pelvic fracture,the study of surgical treatment for the fracture of the pelvic anterior ring is particularly important.This paper aims to make a contrastive analysis of the group of 50 cases of anterior pelvic ring fractures in patients with clinical data,and compare the clinical effect of internal fixation via modified Stoppa approach and minimally invasive parosteal plate osteosynthesis in the treatment of anterior pelvic ring fracture,finally,we want to find simpler and more effective surgical methods for the treatment of the anterior pelvic ring fracture.Methods:The clinical data of 50 patients with anterior pelvic ring fractures admitted to the department of trauma surgery of the Affiliated Hospital of Qingdao University from January 2013 to June 2017 were analyzed retrospectively.All patients were admitted to the hospital to assess the condition accurately,the auxiliary examination was needed to improve the clinical practice,for the critically ill patients,the relevant department consultation should be applied actively and the related complications should be dealt with in time,all patients were examined by iconography before operation to confirm the existence of unstable fracture of the anterior pelvic ring and the classification of fracture,and no severe combined injury was found.All patients were divided into two groups of A and B according to the operation method,including 30 cases with minimally invasive parosteal plate osteosynthesis internal fixation(group A),20 cases with modified Stoppa approach(group B).After the end of clinical treatment,we compared the average operation time,the average intraoperative blood loss,the average hospitalization time between group A and B.The Matta standard was used to evaluate the reduction of fracture,and the Majeed scoring system was used to evaluate the recovery of limb function after operation.The incidence of postoperative complications was recorded at the same time.Finally,the clinical therapeutic effects of these two kinds of surgical approaches were determined by statistical comparison and analysis of the above indexes.Results:The average operation time between group A and B were: 69.0±3.2min,127.0±3.4min,the average hospitalization time between group A and B were: 15.3±0.9d,18.5±0.6d,and the average intraoperative blood loss between group A and B were: 103.2±6.6ml,244.3±9.0ml.The differences among the above three indicators were statistically significant(P<0.05).There was no statistical significance in the excellent and good rate of fracture reduction and limb functional recover and the incidence of complications between the two groups(P>0.05).Conclusion:For unstable anterior pelvic ring fractures,the minimally invasive parosteal plate osteosynthesis internal fixation is more simple,more safety,less injury to the patients,the postoperative recovery cycle of patients is shorter and the clinical curative effect is affirmative.It can be used as the preferred treatment for unstable pelvic anterior ring fracture further promotion in clinical work.
