OBJECTIVE: This study will provide meta-analysis of the efficacy of the improved Stoppa approach and iliac inguinal approach in the treatment of pelvic fractures and acetabular fractures, providing clinicians with more evidence-based medical guidance and significance in selecting surgical procedures.Materials and Methods: Use the databases of China National Knowledge Infrastructure(CNKI),Pubmed,Cochrane Library,VIP database,Wanfang database to search literature, search for modified Stoppa approach and Ilioinguinal approach for pelvic fractures and acetabular fractures. In the English search,"Modified Stoppa approach","Ilioinguinal approach","pelvic fractures",and "acetabular fractures" are combined as keywords. The Chinese search included keywords such as "modified Stoppa approach","Ilioinguinal approach","pelvic fracture",and "acetabular fracture" as keywords or subject terms.Using Review Manager 5.3 for meta-analysis of relevant data in the literature.RESULTS: A total of 18 articles were included in the meta-analysis.A total of 1368 fractures were identified,of which 626 were for the modified Stoppa approach and 742 for the Ilioinguinal approach group.Compared with the Ilioinguinal approach, the modified Stoppa approach has obvious advantages in the treatment of pelvic and acetabular fractures with regard to operative time, complication rate, intraoperative blood loss, excellent hip joint rate, incision length, and anatomical reduction rate of fractures. The difference was statistically significant(P<0.05).CONCLUSIONS: Based on the retrieved literature, this meta-analysis shows that the modified Stoppa approach has significant advantages in the treatment of pelvic and acetabular fractures relative to the iliac inguinal approach, which can reduce intraoperative blood loss, operative time, and increase anatomical reduction.Rate, reduce incision length, increase postoperative hip joint scores, and reduce the incidence of postoperative complications.
