Objective:The aim of the present study was to investigate the incidence of the thyroid dysfunction in patients with acute ischemic stroke and its influence of overt or subclinical thyroid dysfunction on the risk factors of cerebral infarction;to investigate the possible role of the thyroid function in the development of cerebral infarction. Methods:Collected the patients with acute cerebral infarction who were enrolled in neurology department of the second hospital of Jilin University from July 2015 to June 2016.Questioned the patients with the history of medical, alcohol and tobacco,The tests of laboratory, cerebrovascular examination and other necessary auxiliary examination were done.A total of 294 patients diagnosed with first-time acute ischemic stroke were included in the study and categorized for hyperthyroidism(Including overt hyperthyroidism and subclinical hyperthyroidism),hypothyroidism(Including overt hyperthyroidism and subclinical hyperthyroidism)or normal depending on the thyroid function tests. The differences of gender, age, hypertension, diabetes mellitus, heart disease(atrial fibrillation),carotid artery atherosclerosis and biochemical parameters(leukocyte, serum albumin,hyper-sensitive C-reactive protein, trioxypurine, cysteine proteinase inhibitior, triglyceride, total cholesterol,low-density lipoprotein cholesterol,high-density lipoprotein cholesterol, apolipoprotein A,apolipoprotein B,Homocysteine)were analyzed and compared. Results:In the 294 cases of patients with first-time acute ischemic stroke, male 182 cases, female 112 cases;the average age of them was63.59±10.53 years.Of the 294 participants,62(21.09%)were diagnosed with thyroid dysfunction, and there are 56(90.32%)cases with subclinical hypothyroidism and subclinical hyperthyroidismin. The incidence of atrial fibrillation in hyperthyroid group was higher than the normal group, there was significant difference between the two groups(P < 0.05).The incidence of carotid artery stenosis in Hypothyroidism group of was higher than normal group, there was significant difference between the two groups(P < 0.017).The average level of cystatin C in hypothyroidism group is higher than hyperthyroidism, there was significant difference between the two groups(P < 0.05).Conclusions:The relationship between the thyroid dysfunction and the cerebral infarction is close, because the thyroid dysfunction is related to multiple risk factors(atrial fibrillation, carotid stenosis, cysteine proteinase inhibitor C)of cerebrovascular disease.Maybe the thyroid dysfunction is an independent risk factor for cerebral infarction.
