Objective:To observe the effect of whole body vibration training on lower limb spasticity,gross motor function,balance function and adverse reactions in children aged 1~3 years with spastic cerebral palsy.Methods:1.Selection of subjects: In the study,184 children aged 1~3 years with spastic cerebral palsy in Children's Rehabilitation Department of the third affiliated hospital of Zheng Zhou University from Nov 2015 to Nov 2016 were selected,and randomly divided into conventional training group and whole body vibration training group,where there were 150 children completed 24 weeks of rehabilitation treatment and follow-up,including conventional training group 75 cases(n=75)and whole body vibration training group 75 cases(n=75).2.Therapies: The conventional training group received conventional physical therapy,Paraffin therapy and neuromuscular electrical stimulation for 24 weeks.The whole body vibration training group received whole body vibration training on the basis of The conventional training for 24 weeks.3.Evaluation of therapeutic effect: Before treatment,12 weeks and 24 weeks during treatment,The modified Tardieu scale(MTS),the values in the passive state measured by surface electromyography(sEMG)were used to evaluate the degree of spasticity in children.The 88-item version of the Gross Motor Function Measure(GMFM-88),Berg balance scale(BBS)were used to evaluate the gross motor function and balance function.4.The observation of adverse reactions: during and after whole body vibration training,observe whether the children appear crying,discomfort,dizziness,swelling and edema of lower limbs and other adverse reactions,and make records.Results:1.Compared to the baseline:Before treatment,there were no significant differences in the modified Tardieu scale(MTS)score,ankle joint angles R1 and R2 in MTS,the values in the passive state measured by surface electromyography(sEMG),the 88-item version of the Gross Motor Function Measure(GMFM-88)score,Berg balance scale(BBS)score between the two groups(P>0.05).2.Spasticity:The modified Tardieu scale(MTS)score,ankle joint angles R1 and R2 in MTS,the values in the passive state measured by s EMG were significantly improved at 12 weeks and 24 weeks during treatment(P<0.05).The modified Tardieu scale(MTS)score and the values in the passive state measured by sEMG of hamstring and ankle plantar flexor,ankle joint angles R1 and R2 in MTS were significantly improved in The whole body vibration training group compared with the conventional training group at each time points(P<0.05).The modified Tardieu scale(MTS)score and the values in the passive state measured by sEMG of adductor were no differences at each time points(P>0.05)?3.The gross motor function and balance functionThe 88-item version of the Gross Motor Function Measure(GMFM-88)score,Berg balance scale(BBS)score were significantly improved at 12 weeks and 24 weeks during treatment(P<0.05).Berg balance scale(BBS)score were significantly improved in The whole body vibration training group compared with the conventional training group at each time points(P<0.05);The 88-item version of the Gross Motor Function Measure(GMFM-88)score were no differences at 12 weeks during treatment(P>0.05),and significantly improved at 24 weeks during treatment(P<0.05).4.Adverse reactionsDuring and after whole body vibration training,there was temporary crying in a few children but stopped quickly by the parent's appeasement.There were no discomfort,dizziness,swelling and edema of lower limbs and other adverse reactions.Conclusion:1.Compared with the single conventional training,whole body vibration training combined with conventional training can significantly improve the gross motor function and balance function,relieve the Spasticity of hamstring and ankle plantar flexor in children aged 1~3 years with spastic cerebral palsy.2.Whole body vibration training is safety for children aged 1~3 years with spastic cerebral palsy and no serious adverse reactions were found.
