PurposeIn this study,the stability of the implant was measured by measuring the levels of Osteoprotegerin(OPG)and receptor activator of nuclear factor kappa B ligand(RANKL)in gingival crevicular fluid(GCF)and perio-implant crevicular fluid(PICF)in periodontal tissues at different time points.Implantation quotient value.The relationship between OPG,RANKL level and OPG / RANKL ratio and ISQ was analyzed.MethodScreening patients from May 2011 to May 2016 to our hospital for stomatology.After screening,a total of 78 patients were selected,including 43 males and 35 females,the patient age range between 20-62 years old,the average age of 41.6 ± 14.2 years old,.A comprehensive assessment of the planting site,according to the patient's imaging and specialist examination results,to develop a detailed planting plan,a total of 78 implanted implant were placed.All patients were treated with standard surgical procedures,to complete Straumann tissue-leve implant implantation.The retention rate of the implants was determined according to the criteria proposed by Buser.Preoperative and postoperative absorption of GCF and PICF with absorbent paper,measured GCF and PICF weight.The stability of the implants was measured using an implant stability meter(Osstell ISQTM(Sweden)).At the time of examination,the implant stability quotient(ISQ)was recorded on the near,buccal and lingual sides of the implant.The ISQ values of the ISQ values were used as the ISQ values of the implants.The patients were followed up at 1,2,3,4,6,8 and 12 weeks.PICF and GCF were measured and ISQ values were measured.In addition,the ELISA method was used to detect the levels of osteoprotegerin(OPG)and nuclear factor kappa B receptor receptor(RANKL)in the collected GCF and PICF samples.The changes of OPG,RANKL and OPG / RANKL were correlated with the relationship between OPG and RANKL at the postoperative and weekly review.ResultAfter screening,the final selection of a total of 78 people,including 43 female patients,35 males,the average age of 41.2 ± 12.9 years old.A total of 78 implants were implanted.The implant diameter is divided into 3.3mm,4.1mm and 4.8mm three,the corresponding number of planting were 13,36 and 29 people.After surgery,all implants were healed,no significant gum swelling and other periodontal inflammatory response.78 implants did not find the phenomenon of implant breaking,loosening and shedding.The radiographic examination showed that there were no transmissive shadows and bone resorption around the implant,and the implant retention rate was 100%.Repeated measurement of variance between OPG and RANKL detected at different time points showed that the content of OPG in GCF and PICF increased in the second week after operation,and the OPG in PICF was the most significant,which was significantly higher than that in other OPG level at the time point(P <0.05).At the second week,the content of OPG in GCF was also significantly higher than that at other time points(P <0.05).The results showed that there were significant differences in the stability of the implant at different time points.The implant stability of the mandibular tooth was generally higher than that of the maxillary posterior part,and the implant stability was significantly higher than that of the maxillary posterior teeth.Especially at 3 weeks,6 weeks,8 weeks and 12 weeks,the difference between the two groups was significant(P <0.05).The differences of OPG between GCF and PICF at the same time point were also analyzed.The results showed that the difference was not statistically significant(P> 0.05).While the content of RANKL in GCF and PICF showed different trend.The RANKL level in GCF was significantly lower than that in week 2(P <0.05),while RANKL level in PICF was the highest at week 3,and the levels of RANKL in GCF were significantly lower than those in week 1,6,8 and 12 weeks,RANKL levels were significantly lower than the first 3 weeks(P <0.05).The difference of RANKL between GCF and PICF at the same time points were analyzed.The results showed that the difference was not statistically significant(P> 0.05).Based on the OPG and RANKL data at different time points,the OPG / RANKL ratios at GCF and PICF at different time points were calculated.At week 1,the OPG / RANKL ratio in PICF was significantly higher than the OPG / RANKL ratio in GCF(P = 0.034 <0.05).There was no significant difference between the two groups at other time points.The ISQ values at different time points were compared with the ISQ measurements at 63,and the ISQ was slightly increased at 1 week postoperatively,then decreased to a minimum of 62.2 ± 0.9 in the fourth week and then gradually increased The Statistical analysis showed that the ISQ values were significantly higher in the first week,the second week,the 6th week,the 8th week and the 12 th week than in the fourth week,the difference was statistically significant(P <0.05).The change of OPG and RANKL in GCF was more correlated with the change of ISQ.When the level of OPG was increased,the level of ISQ decreased,while the change of RANKL showed the same trend as that of ISQ.When RANKL level decreased,ISQ level decreased When the RANKL level rises,the ISQ level also increases,while the OPG and RANKL changes in the PICF appear earlier than the ISQ changes.When the ratio of GCF rises,the ISQ value decreases and the ISQ value increases when the ratio decreases.The change in OPG / RANKL ratio in PICF is still earlier than the change in ISQ value,1 week earlier than the change.Conclusion1.Due to the biological effects of OPG and RANKL,the trend of OPG and RANKL was different during the recovery process after implantation.And therefore compared with the trend of ISQ also showed a different relationship.OPG and ISQ trends in the opposite,and RANKL and ISQ changes in the same trend.2.The changes in OPG and RANKL in GCF are earlier than PICF,so GCF is more sensitive than PICF.
