The Clinical Study On Chuanglingye Improved The Local Inflammatory Factors Of Plasma Cell Mastitis

Objective:To investigate the effect of ChuangLingYe on local proinflammatory cytokinein of plasma cell mastitis(PCM)with abscess.To evaluate the therapeutic effect of ChuangLingye on local inflammatory response of plasma cell mastitis(PCM)with abscess.Methods:Twenty patients with PCM who met the recruitment criteria were randomly divided into two groups:the treatment group(n = 10)and the control group(n=10).For the control group,direct excision was performed for abscess drainage and 30ml of saline was applied for irrigation.Medical gauze with scutellaria oil was used for drainage.Wound dressing was changed once per 2-3 days.For the control group,direct excision was performed for abscess drainage and 30ml of ChuangLingYe was applied for irrigation.Medical gauze with ChuangLingYe was used for drainage.Would dressing was also changed once per 2-3 days.The treatment and observation period for the two groups of patients lasted for 1 month.Treatment D1,D14,D30 fill out the evaluation forms for the clinical symptoms observations and collected granulation tissue samples from patients' wound surface.Part of the granulation tissue was used for homogenate to assess the levels of IL-2,IL-4 and IL-6 and part of the granulation sample was used for immunohistochemistry to investigate the expression of IL-2,IL-4 and IL-6 and the evaluation of the inflammatory response.Results:The results showed that:(1)Clinical symptom score:After the 1-month treatment,the treatment group have 2 cases recovered,5 cases are markedly improved,3 cases are effective,the total effective rate of the treatment group is 100%,the control group only 1 case is effective,the total effective rate was 10%,the comparison between them was statistically significant(P<0.05),the effect is remarkable.And the post treatment score were significantly lower than the start treatment score(P<0.05).The symptom score of the treatment group was significantly lower than the score of control group(P<0.05),which has statistical significance.Although the treatment score of control group were decreased at the beginning of the treatment,but after the D14 the integral were rised,there is no obvious difference between the start and post treatment score(P>0.05).And compared with the treatment group's score,the control group's score have no difference(P>0.05).(2)According to the homogenate evaluation of the granulation tissue sample,the IL-2,IL-4 and IL-6 levels were all lower in the treatment group than those in the control group(P<0.05),which showed statistical significance.(3)Pathological Immunohistochemistry Evaluation:The expression of IL-2 and IL-4 in the treatment group was obviously inhibited,the positive expression of inflammatory cells in the control group had no significant difference before and after treatment(P>0.05).(4)Pathological Inflammation Rating:Compared with the saline group,the plasma cells of the treatment group are decreased significantly(P = 0.01),the control group had no difference(P>0.05).Conclusion:1.ChuangLingye can reduce the local inflamnatory symptoms,such as pain relief,improve the local skin changes and make the tumor narrowed etc.2.ChuangLingYe can regulate IL-2,IL-4 and IL6.It can inhibit the inflammatory response of plasma cell mastitis and reduce the proliferation of granulation tissue and promote overall wound healing.
