| Objective: Vitiligo is a common skin and mucous membrane pigment degeneration disease,Belonging to one of the common diseases of dermatology.The cause of Vitiligo is complex,and its pathogenesis is unclear.The prevalence rate is 0.1~2.7% in our population.Vitiligo often occurs in the exposed and friction parts,affecting the patient's aesthetic and interpersonal relationships,to patients with greater psychological barriers.Vitiligo treatment is more difficult,with the gradual development of medicine,its treatment is also increasing.In recent years,light therapy and photochemical therapy gradually rise,PUVA is one of the more applied therapy,it is psoralen and long wave UVA in combination,although its efficacy has been clinically recognized,but its adverse reactions can not be ignored.UVA1(340-400 nm)is a band of longwave UVA,and its skin penetration is stronger.In this study,through the observation of three different treatment methods of patients with stable vitiligo,to explore the efficacy of UVA1 combined with topical Compound kaliziran tincture in stable vitiligo,to provide a new program for the clinical treatment of vitiligo.Methods: In this study,We collected from September 2016 to December 2016,in Department of Dermatology clinics of the Second Affiliated Hospital of Dalian Medical University Hospital in line with the inclusion criteria and exclusion criteria of vitiligo patients,and 26 patients were enrolled in the study.During the course of treatment,2patients withdrew from the study for personal reasons,so a total of 24 cases were selected.Among them,12 males and 12 females,aged 14 to 57 years,duration of 6months to 23 years.Including a total of 72 lesions,lesions distributed in the head,face,neck,chest,abdomen,back,hand and foot,limbs,etc.,in line with their own controltest requirements.24 cases of vitiligo patients with adjacent or symmetrical site of 3white spots,were randomly divided into the Compound kaliziran tincture group,UVA1 group and Combined group,to take their own control study methods for the treatment of skin lesions to observe the situation.Compound kaliziran tincture 3 to 4 times a day,UVA1 irradiation 2 times a week.In the treatment of 8 weeks,12 weeks later,according to the efficacy evaluation criteriathe of vitiligo to determine the criteria for evaluation,including recovery,markedly,improved,ineffective.After calculating the lesion,calculated the total effective rate,the efficacy of the three groups was statistically compared.And after 12 weeks of treatment,the different parts of the skin lesions in Combined group were statistically analyzed,and the efficacy was compared.Results:1.Comparison of clinical efficacy: After 8 weeks of treatment,the total effective rate of Compound kaliziran tincture group was 33.33%,the total effective rate of UVA1 group was 37.50% and 58.33% in the Combined group.There was no statistically significant difference in the three groups(P> 0.05).After 12 weeks of treatment,The total effective rate of Compound kaliziran tincture group was 50.00%;the total effective rate of the UVA1 group was 54.17%,and the total effective rate of the combined group was 79.17%.There was no statistically significant difference between Compound kaliziran tincture group and UVA1 groups(P > 0.05),The efficacy of the Combined treatment group was higher than that of Compound kaliziran tincture group and UVA1 group,the differences was statistically significant(P < 0..05).2.Comparison of the efficacy of different parts of skin lesions in Combined treatment group: After 12 weeks of treatment,the efficacy of facial and neck was significantly higher than that of limbs and hand and foot,the difference was statistically significant(P<0.05),the efficacy of trunk was higher than that of hand and foot,the difference was statistically significant(P <0.05),and there was no significant difference in the other parts(P> 0.05).Conclusion: After 12 weeks of treatment,the total effective rate of the combination of Compound cardurization and UVA1 is 79.17%.The combined effect ofCompound kaliziran tincture and UVA1 is significantly higher than that of Compound kaliziran tincture,UVA1 applied separately,and less adverse reactions,which clinical efficacy and location are related,so it's a new treatment program for stable stage of vitiligo patients,worthy of promotion. |
