Background:With the development of modern society,the masses on the decline of material dependence,people pay more and more attention to their own image,the demand for beauty is more and more high,hair loss disease seriously affects the personal image,so more and more patients due to hair loss to see a doctor.Androgenetic alopecia(AGA)is one of the most common hair loss disorders.It can occur in both men and women.It is a progressive pattern of hair loss,characterized by a progressive decrease in hair density.The onset age of AGA is usually between 30 and 40 years old,but in recent years,its onset trend is gradually younger.The current FDA-approved drugs for the treatment of AGA are oral finasteride and topical tincture of minoxidil,both of which have some side effects.As AGA is a progressive disease with a long disease cycle,there is no specific radical treatment,and many patients are difficult to accept long-term oral drug therapy.More and more patients advocate natural,green and safe traditional Chinese medicine methods to prevent decay,and the search for safe and effective external treatment has attracted more and more attention.Compound Siyadan Tincture is a Compound of Uygur herbal medicine that can be used to treat AGA.Previous studies have shown that Compound Siyadan Tincture is safe and effective in the treatment of AGA,and it is difficult to objectively evaluate its clinical efficacy by early clinical research methods.Objective:In order to standardize and evaluate the efficacy,safety and germinal mechanism of Compound Siyadan Tincture in the treatment of androgenic alopecia,this study evaluated its efficacy and safety through clinical study,and observed its germinal mechanism through animal experiment.To provide evidence support for the treatment of androgenic alopecia with Compound Siyadan Tincture.Methods:Thirty AGA patients were enrolled in the study and received Compound Siyadan Tincture twice a day for 6 months.Hair count,hair density,hair diameter,and vellus hair ratio were evaluated using the trichoscan image analysis system before and after autocontrol,and standardized clinical photos were taken to evaluate the improvement of the patients.Clinical symptoms,quality of life score(DLQI)and patient satisfaction were recorded,adverse events in all cases were recorded,and the clinical efficacy and safety of Compound Siyadan Tincture in the treatment of AGA were standardized evaluated.A model of androgenic alopecia induced by dihydrotestosterone(DHT)was established in C57BL/6 mice.Forty 7-week-old male C57BL/6 mice were randomly divided into 4 groups with 10 mice in each group.They were:A:negative control group;B:model group;C:experimental group(Compound Siyadan Tincture group);D:positive control group(5%Minoxidil Tincture group).The hair growth of mice was evaluated by skin color change,dermal thickness.morphology and number of hair follicles,and the effect of Compound Siyadan Tincture on the hair growth of C57BL/6 mice with androgenic alopecia induced by DHT was observedResults:The clinical trial showed that the number of hair was increased by 13.750±12.889 hairs/field after treatment compared with before treatment,and the comparison between the total number of hair before and after treatment was 146.11 ±21.205 vs 159.86±22.561 hairs/field,the difference was statistically significant(P<0.001).In terms of hair density,it was 13.125±12.29205 hairs/cm2 after treatment and 139.454±20.2404 hairs/cm2 vs.152.579±21.532 hairs/cm2 before and after treatment.the difference was statistically significant(P<0.001).The average diameter of hair before and after treatment was 58.7±7.03?m vs.59.32±8.046?m(P>0.05).Although the difference before and after treatment was not statistically significant,it was suggested that Compound Siyadan Tincture could make hair diameter thickened to some extent.In terms of the total number of hair follicles and the number of hair follicle units,there were statistically significant differences in the number of hair follicles and the number of hair follicle units containing 2.3,4 or more hairs before and after treatment(P<0.05),and both of them were better after treatment than before treatment.In addition,Compound Siyadan Tincture can improve scalp inflammation,scalp grease,scalp itching,dandruff and scalp folliculitis to a certain extent,and improve the quality of life of patients with good safetyThe results of animal experiments showed that according to the hair growth and trichoscope performance of each group in different periods,in terms of hair growth,the negative control group>the experimental groupgroup?positive control group>model group,indicating that the androgenetic alopecia model of C57BL/6 mice induced by DHT in this experiment was successfully established.According to the results of HE staining of the back skin tissue of mice on the 24th day after depilation,in terms of the total number of hair follicles,the total number of hair follicles in the negative control group was 87.97±20.94,the total number of hair follicles in the model group was 65.35±20.34,the total number of hair follicles in the experimental group was 86.28±26.42,and the total number of hair follicles in the positive control group was 84.23±25.38.The comparison of the negative control group,the experimental group and the positive control group with the model group had statistical significance respectively,but the pairwise comparison among the three groups showed no statistical significance.The dermal thickness of the negative control group was 312.23±52.59?m,the model group was 290.35±44.10?m,and the experimental group was 314.07±45.01?m.The dermal thickness of the positive control group was 318.42±60.46?m,and the comparison between the negative control group,the experimental group and the positive control group was statistically significant compared with the model group,but the pair comparison among the three groups showed no statistical significance.The results showed that Compound Siyadan Tincture can reverse the effect of DHT intervention to a certain extent,shorten telogen phase,lead to an increasing number of hair follicles in the anagen phase,and promote telogen-to-anagen transition in C57BL/6 mice,and achieve the effect of promoting hair growth,and its efficacy is similar to that of 5%minoxidil tincture,with good safety.Conclusion:Clinical studies showed that Compound Siyadan Tincture can promote hair regeneration,increase the total number and density of hair,make hair thickening,improve scalp inflammation,scalp grease,scalp itching,dandruff,scalp folliculitis to a certain extent,with good safety.The results of animal experiments showed that the model of androgenic alopecia induced by DHT in C57BL/6 mice was successfully established.Compound Siyadan Tincture could promote the hair growth of DHT-induced androgenic alopecia model in C57BL/6 mice,increase the dermal thickness and the total number of hair follicles,shorten the rest period of hair follicles in C57BL/6 mice,and promote the hair follicles in C57BL/6 mice to enter the growth period in advance,which was similar to the effect of 5%minoxidil tincture.Good security.Compound Siyadan Tincture is safe and effective in the treatment of androgenetic alopecia,which is worthy of further application in clinical practice.
