| Background: Hemorrhoids are one of the most common disease daily treated in surgical clinic.Third and fourth degree piles were always could not be cured without surgical treatment.However,pain after conventional excision cold bot be controlled easily with oral medication.New surgery such as Ligasure assisted pile excision and Doppler-guided hemorrhoidal artery ligation and rectoanal repair(HAL-RAR)provide were widely used in surgical clinics,These two procedures not only have better cure rate,but also provide less surgical pain than conventional milligan-morgan hemorrhoidectomy Research purposes: Compare the advantages and disadvantages between the two different procedures.Which most suitable in out patient surgery for hemorrhoids above grade 3.Method : We choose 80 patients who suffered from the grade 3~4 hemorrhoids during the January.2015 to June.2015.Adopted the method ofRandomized controlled trials,randomly divided the 80 patients into two groups,40 patients in experimental group treated with DG-HAL,and 40 patients in control group treated with ligasure assisted pile excision.Compare the differences in surgical wound pain,cure rate and satisfaction between to two groups.Results : Though postoperative cure rats and anal redundant skin tags were poor in experimental group.However,postoperative pain is far worth in control group.And postoperative satisfaction is better in the experimental group.(P <0.001)Conclusion: DG-HAL takes advantages in both high cure rates and less postoperative pain,most patients did not return to clinic again due to surgical pain.Though there is higher cure rates in ligasure group,but in postoperative pain and return to clinic in.24 hours,satisfaction,it is not good as the DG-HAL group.Thus we recommended DG-HAL as the first choice for hemorrhoids above grade 3 in out patient surgical clinic. |
