Purpose:30 cases of hospitalized patients with severe hemorrhoids were involved in this study.To observe advantages and disadvantages of two surgery in the treatment of severe hemorrhoids by Comparative observation.The two surgery is PPH and Doppler guided hemorrhoidal artery ligation joint PPH,one more aim is to explore the mechanism.Methods:30 hemorrhoids patients are divided into two groups(n=15) randomly which internal hemorrhoids are level Ⅲ-Ⅳ.The treatment group using PPH combined with Doppler guided hemorrhoidal artery ligation,the control group using PPH operation.To observe the clinical curative effect, in two groups including intraoperative blood loss,abdominal tugging,hospital stay,edema, postoperative pain,postoperative blood,postoperative anal discomfort,retention of urine,anal function and other complications.Result:Experimental control groups of patients in the postoperative complications such as pain, edema, urine situation, falling up anal discomfort, anal narrow has no significant difference (P>0.05); its recent clinical efficacy, hospital stay, postoperative recovery time has no significant difference (p>0.05); intraoperative anastomotic bleeding experimental group is less than the control group (p= 0.03<0.05);There is no difference in anal function, fecal urgency in 3 months.(P>0.05), There is no hemorrhoids prolapse after 3 months in treatment group, on the other hand there are two cases in the control group, two groups were significantly different (P= 0.035<0.05); The research shows that the test group and the control group were significant differences in overall efficacy in 3 months, experimental group had no complication such as bleeding hemorrhoid prolapse and other symptoms, the control group have 3 patients with mild bleeding after defecation, there is one patient with hemorrhoids prolapse (treatment group is superior to control group, p=0.0350.05); both of two groups didn’t increase more complications after surgery besides there is no anal infection, rectovaginal fistula intestinal perforation, pelvic abscess crisis.Conclusion:PPH joint Doppler guided hemorrhoidal artery ligation is a satisfactory clinical procedure in the treatment of severe hemorrhoids.The surgery didn’t increase more complications and improves the long-term efficacy by comparing with the traditonal PPH surgery.
