Clinical Validation Of “Physiological-Psychosocial-Ethical” Treatment Model

Background The concept of physiological integration medical model has been advocated for many years,but has not established a clinically implemented,verifiable program.With the guidance of simple,quantitative and operational,we try to establish “Physiological-Psychological – Ethical” comprehensive diagnosis and treatment model,in order to improve the efficacy,alleviate doctor-patient conflict and promote social harmony,it should have academic value and application prospect.The project team combined with the research results of medical model transformation at home and abroad,experienced nearly three years of clinical practice,has initially completed comprehensive diagnosis and treatment model index and evaluation manual.And,we have established the operation manual of comprehensive diagnosis and treatment model,in cooperation with many integrated hospitals.Objective Cardiovascular disease is a high prevalence of " psychosomatic disease".Therefore,the established comprehensive diagnosis and treatment model and evaluation manual and comprehensive diagnosis and treatment model operation manual,the effectiveness and feasibility of the “Physiological-Psychological– Ethical” comprehensive diagnosis and treatment model in the comprehensive hospital.Methods Using the established ?The Operation Procedure of comprehensive diagnosis and treatment model?and?comprehensive diagnosis and treatment model assessment and Indicators Manual?,we try to test if comprehensive diagnosis and treatment model is actionable and useful.We have collected 179 cases of all,of which 149 were valid.Divided the patients who meet the hide and sea conditions into intervention group and control group randomly.The intervention group patients adopt “Physiological-Psychological– Ethical” comprehensive diagnosis and treatment model for treatment(add psychological intervention and implementethical principle in the process of treatment),and the control group adopt conventional diagnosis treatment Use spss21.0 to analyze the data.Results After treatment,the significant efficiency,effective rate and inefficiency rate of unstable angina pectoris in the intervention group were 61.4 %,37.1 % and 1.4 % respectively.the efficiency,efficiency and inefficiency were 46.8 %,43 % and 10.1 % respectively.The significant efficiency,effective rate and inefficiency of abnormal ECG in intervention group were 44.3 %,44.3 % and 11.4 %,respectively.the efficiency,effective rate and inefficiency rate were 25.3 %,54.4 % and 16.1 %,respectively.the ? 2 value was 6.465,p < 0.05.In psychological indicators,the scores of two groups of patients and each dimension were tested t test.The results showed that the five factors of depression,irritability,anxiety,life stress and life events in the intervention group were lower than before treatment,the t value was 4.11,6.44,5.11,3.58 and 3.35,respectively;After treatment,the scores of mental symptoms,life style and social situation were lower than before treatment,the t value was 5.96,5.76 and 4.91,respectively.The scores of depression,easy irritation,anxiety and life events in the control group were lower than before treatment,the t value was 2.57,2.47,2.55 and 2.08 respectively;After treatment,the scores of mental symptoms,life style and social situation were lower than before treatment,the t value was 2.70,3.66 and 2.08,respectively.There was no significant difference in the scores between the two groups.The results showed that the score of depression and anxiety of the intervention group was lower than the control group,and t value was 2.30 and 2.05,respectively.On ethical indicators,the service attitude and communication,the explanation of the disease related factors,the explanation of disease related knowledge,the interpretation,acceptance,cost awareness,discharge attention matters were higher than the control group,and the difference was significant.After treatment,the score of the intervention group of the total impression,overall satisfaction and total score of hospitalization were higher than those of routine treatment,the t value was 7.40,7.65 and 9.49,respectively.ConclusionCompared with the conventional diagnosis and treatment model,the implementation of“Physiological-Psychological– Ethical” comprehensive diagnosis and treatment model can effectively alleviate the angina symptoms and ECG abnormalities.Compared with the conventional diagnosis and treatment model,“Physiological-Psychological– Ethical” comprehensive diagnosis and treatment model can significantly improve the depressive symptoms and anxiety symptoms of patients.The ethical practice in the integrated diagnosis and treatment mode of "Physiological-Psychological– Ethical" has improved the patient experience,improved the patient satisfaction and promoted the harmony of doctor-patient relationship.The clinical verification of " Physiological-Psychological– Ethical" integration model shows that the model is effective and feasible.
