Effect Of Stellate Ganglion Block Combined With The Trigeminal Pulsed Radiofrequency Treatment For The Facial Postherpetic Neuralgia

Objective:To evaluate the curative effects and the safety of stellate ganglion block with trigeminal-pulsed radio frequency treatment for the post herpetic neuralgia and facial postherpetic neuralgia.And to investigate the changes of serum IL-2 and IL-6 levels before and after treatment.Methods:Select 60 patients with postherpetic neuralgia,and divide to three groups depending on different treatments,with 20 patients each.The patients of group A received patient-controlled stellate ganglion block,and those in group B received trigeminal pulsed radiofrequency treatment.The patients of group C received stellate ganglion block with trigeminal-pulsed radio frequency treatment and doctors extract the serum of patients before and after the treatments as reserve.The routine medications of those three groups of patients are the same.Observing after one weak,one month,six months,and twelve months of treatments,the visual analogue scale(VAS)and the quality of life(QOL),the effective rate,the dosage of analgesic,the level,the frequency and the duration of facial pain,and If there is any complications.Also use the radioimmunoassay method to determine serum levels of IL-2 and IL-6 before and after treatments.Results:The scores of VAS in group A(7.51 ± 0,92 vs 3.31 ± 0.40),group B(7.51 ± 0.80 vs 2.81 ± 0.45)and group C(7.63 ± 0,68 vs 2.06 ± 0.55)were significantly decreased before and after treatment for 6 months(P<0.05)Statistical differences(P<0.001).The scores of QOL in group A(34.64 ± 2.83 vs 72.62 ± 2.07),group B(33.51 ±2,47 vs 72.84 ± 2.40)and group C(32.12 ± 3.00 vs 79.82 ± 3.79)were compared before and after treatment for 6 months.Were significantly increased and statistically significant(P<0.001).The percentage of pain relief in group C was significantly higher than that in group A(25%)and group B(30%)(P<0.05).The duration of pain was significantly higher in group A(10.54 ± 0.31 vs 4.38(10.41 ± 0.43 vs 3.41 ± 0.67)in group B(10.16 ± 0.49 vs 3.74 ± 0.53),which was significantly lower than that before operation(P<0.05),and was statistically significant.The levels of serum interleukin-2 in each group were higher than those before operation(group A:2.49 ± 0.81 vs 2.56 ± 0.72;group B:2.52 ± 0.79 vs 2.55±0.82;group C:2.54 ± 0.78 vs 2.61 ±0.78 ng/ml)(group A:244.65 ± 48.15 vs 243.51 ±48.12;group B:243.62 ± 47.95 vs 242.61 ± 47.92;group C:242.38 ± 46.95 vs 241.01± 2 pg/ml),and the level of interleukin-6 was significantly higher than that of group A(P<0.05)46.85),but the decrease was not significant(P>0.05).Conclusion:Trigeminal ganglion radiofrequency and stellate ganglion combined with the treatment of facial ganglion block and the trigeminal impulse radio frequency of these two methods for the treatment of facial PHN effect has been significantly improved.And the above treatment method can not significantly improve the serum PHN patients serum interleukin 2 and interleukin 6 levels.
