Backgrounds and Objectives:Spontaneous intracerebral hemorrhage(sICH)have the percentage of 20%-30%in the acute cerebrovascular diseases,the incidence is only second to ischemic cerebrovascular disease.It is a major reason of disability and death for the patient.The related researches of ischemic stroke etiology classification is more common while the cause of hemorrhagic cerebrovascular diseases classification is relatively few.The main brain Imaging examination for diagnosis of craniocerebral hemorrhage is computed tomography(CT),followed by cerebral magnetic resonance imaging(MRI).Magnetic resonance angiography(MRA),computed tomography angiography(CTA)and digital subtraction angiography(DSA)can help the diagnosis of cerebral hemorrhage etiology,but the number of patients who do further imaging studies is still small.Intracerebral hemorrhage often occur in the elderly,but also have a trend of getting younger over this years.The etiology,risk factors and clinical manifestations vary in young and aged people.According to different causes and risk factors,we can reduce and prevent the occurrence of cerebral hemorrhage effectively.Evaluate patients etiology classification and risk factors help to choose effective treatments,prognosis methods and the secondary prevention strategies.The purpose of the research is that through the collection and analysis of clinical data of cerebral hemorrhage patients to summarize the common etiologies and risk factors,improve the clinicians comprehensive understanding of the disease,provide the basis for the system diagnosis and treatments of brain hemorrhage.Methods:Retrospective collection of patients admitted in our hospital neurology wards during the period of 2011.01 to 2016.12.The informations of 442 cases,such as age,sex,etiologies,bleeding locations,life style(smoking,drinking,drug use etc.),blood lipid,blood sugar,liver and kidney function,blood coagulation function,virus series and imaging examination results were gathered for medical statistics analysis.Summarize the differences in two groups as for age and gender aspects,bleeding location,clinical manifestations and different common causes and risk factors.Results:1.The study found that all of the patients with cerebral hemorrhage,the average onset age of men is earlier than women.The young cerebral hemorrhage patients accounts for 14.3%of all cerebral hemorrhage cases.More men than women in the two groups of patients with cerebral hemorrhage,especially in the youth cerebral hemorrhage group.This difference is statistically significant(p<0.05).2.As for bleeding site,the most common site is the basal ganglia region,followed by cerebral lobe,cerebellum,brainstem,multiple locations and ventricle.Basal ganglia,and cerebral lobe are still the most common bleeding locations in both groups,and cerebellar hemorrhage is more common in the elderly cerebral hemorrhage.3.According to SMASH-U,hypertension is the most common reason in two groups.There are certain differences in other pathogenies,the undetermined and abnormal vascular structures occupy a larger proportion in the youth cerebral hemorrhage,while amyloid cerebrovascular disease is the second common cause for intracerebral hemorrhage in the elderly.4.Hyperhomocysteinemia and dyslipidemia are no significant differences in both groups(p>0.05).But the male,history of smoking,alcohol intake and family history are obviously different in two groups and more common in the youth,while the history of hypertension,cerebrovascular disease,diabetes,coronary heart disease are more common for the aged(p<0.05).5.The study found that limb weakness and glossolalia showed no statistically significant difference in two groups(p>0.05),while headache and convulsion of limbs are common for the young,the other clinical manifestations are common in the elderly(p<0.05).6.Only 17.6%of all patients conducted cerebrovascular examination.MRA is the most commonly used to blood vessels inspection in both groups.DSA are common in young people and CTA in elderly patients.Overall,there were seven patients died in-hospital,1 case for youth and others for elderly group.Conclusion and significance:Cerebral hemorrhage etiologies,risk factors and clinical manifestations are various.This need to be more examinations to determine the etiology.There are much differences in many respects for the young and elderly cerebral hemorrhage,analyzing the differences between the two is helpful to guide clinical treatments and prevention.
