Observation On Early Clinical Efficacy Of Two-Marvel Two-Living Decoction For Treatment Of Rheumatoid Arthritis After Arthroscopic Synovectomy

Objective To explore the early clinical efficacy of Two-Marvel Two-Living Decoction(er miao er huo tang)for treatment of rheumatoid arthritis patients after arthroscopic synovectomy,compare it with the efficacy of arthroscopic synovectomy in combination with conventional western medicine,and then provide objective evaluation of the clinical efficacy and safety of Two-Marvel Two-Living Decoction for treatment of rheumatoid arthritis.Methods 60 patients diagnosed with rheumatoid arthritis causing swollen knee joints were selected and received arthroscopic synovectomy.The patients were divided into the control group and the observation group,30 patients in each group.After the procedure,the control group received conventional western medicine to reduce swelling,decrease pain,and improve other rheumatic symptoms while the observation group took Two-Marvel Two-Living Decoction.Two groups were observed and assessed for Visual Analog Scale(VAS)pain score,the degree of knee joint swelling,the Hospital for Special Surgery(HSS)knee score,the level of high-sensitivity C-reactive protein(CRP),Rheumatoid factors(RF),and the amount of proteoglycan(PG)in joint fluid and the efficacy was evaluated according to evaluation standards of TCM efficacy.Results On the 7th day after the surgery,the observation group showed significantly lower VAS score and less postoperative swelling than the control group(P<0.05),however,on the 3rd day after the surgery as well as two weeks after the surgery,the difference of the two groups was not statistically significant(P>0.05).Within the two groups,both of them showed significantly lower blood CRP,RF,and PG in joint fluid before the surgery and two weeks after the surgery(P<0.01);moreover,the observation group showed significantly lower CRP,RF,and PG than the control group two weeks after the surgery(P<0.05).Two weeks after the surgery,the observation group and the control group respectively showed the improvement rates of 80.0%and 73.3%in terms of HSS scores,which was statistically significant(P<0.05).On the 14th day after the surgery,the improvement rates of the observation group and the control group were respectively 86.6%and 83.3%in TCM,which was statistically significant(P<0.05).Conclusion When used after arthroscopic synovectomy,compared with conventional western medicine,Two-Marvel Two-Living Decoction reduced the levels of CRP,RF,and PG more significantly,relieved patients' pain and swelling more quickly and,by improving patient satisfaction and compliance,realized better functional recovery of knee joints,thus improving clinical efficacy.
