Objective: Knee osteoarthritis(KOA)is one of the most common degenerative joint disease,fumigation therapy is more and more attention in the treatment of KOA,but there are some disadvantage the traditional fumigation therapy,such as it is open,and its' time temperature can not control.To solve theses problems,we research and develop the closed device of fumigation for the knee joints,and make it with Duhuojishengtang to fumigate the knees of elderly cynomolgus monkeys which fall in natural degeneration KOA,detecting the influence to the degenerative cartilage cells of elderly cynomolgus monkey's knee joint in Notch1,MMP-13,and COL-? expression,in order to explore the effect of closed device of fumigation for the knee joints to treat KOA,and the effect and mechanism of action to cure KOA by using fumigation of Duhuojishengtang.Methods : The first,design and develop the closed device of fumigation for the knee joints,produce a prototype,and apply for a patent.The second,select 12 elderly cynomolgus monkeys with natural degeneration of KOA,were randomly divided into experimental group and control group,blank control group,each group of four monkeys.The experimental group use the closed device of fumigation for the knee joints combined with Duhuojishengtang to fumigate,the control group using traditional fumigation combined Duhuojishengtang,the blank control group of traditional fumigation processing with saline.Put the animals to death intervented them after 8 weeks,make pathological section from the degenerative cartilage of elderly cynomolgus monkey knee joint,mankin score after microscopically observed the pathological changes,finally,we used Notch1,MMP-13,COL-? to immunohistochemical dye the degenerative cartilage cells of elderly cynomolgus monkey's knee joint,to observe their expression.Results:(1)We have completed the preliminary design and manufacturing entity model of the closed device of fumigation for the knee joints basis of integration of traditional fumigation therapy and modern biological science and technology,then,we have produced a prototype and declared a national patent,patent number(ZL 2013 2 2013 X).(2)In HE staining: the experimental group and control group average of Mankin score is lower than the blank control group,there was significant difference in each group(P<0.05).the experimental group average of Mankin scoring average is lower than the control group,there was significant difference between groups(P<0.05).In immunohistochemical observation,the expression of Notch1,MMP-13 in the experimental group and control group is lower than the control group,there was significant difference between groups(P<0.05).the expression of Notch1,MMP-13 in the experimental group is lower than the control group,there was significant difference between groups(P<0.05);But,the expression of COL-? in the experimental group and control group is higher than the control group,there was significant difference between groups(P<0.05).the expression of COL-?in the experimental group is higher than the control group,there was significant difference between groups(P<0.05).Conclusion:(1)There are some advantage such as constant temperature,closed,controllable in the closed device of fumigation for the knee joints compared with traditional fumigation;(2)Duhuojishengtang fumigation therapy can adjust in the knee joint cartilage Notch1,MMP-13,COL-? expression,and inhibit destruction of the articular cartilage,to effectively delay the knee joint degeneration,achieve its therapeutic effect.The closed device of fumigation for the knee joints has better therapeutic effect of KOA than traditional fumigation.
