Objective:Maxillary sinus perforation is a common complication of alveolar surgery.If the perforation cannot be timely and effective,the treatment will lead to maxillary sinus infection.Traditional flap transfer repair of maxillary sinus perforation treatment program will take secondary trauma to patients.Concentrated growth factor(CGF)is rich in concentrated fibrin and platelets,which can promote the repair and regeneration of tissue defects.This research studies the repairing maxillary sinus perforation cases of CGF and Flap Transfer Technique,evaluates the clinical effect of concentrated growth and offers a optimal clinical treatment for repairing maxillary sinus perforation.Methods:Firstly,there is a retrospective analysis of 15 patients(CGF group)who had maxillary sinus perforation repair(outpatient and inpatient)in Department of Oral and Maxillofacial Surgery of Shandong Provincial Hospital from December 2006 to January 2016.Of this,five cases are maxillary sinus floor defects and 10 cases are maxillary sinus perforation after tooth extraction.Secondly,there are 15 patients who were treated with flap transfer technique(buccal periosteal flap glide,palatal visceral periosteal flap and buccal pad transplantation)as a control group(flap transfer group).Among them,6 cases are maxillary sinus bone defect caused by apical lesions of maxillary posterior teeth and 9 cases are maxillary sinus perforation after extraction.There will be comparisons of(1)Surgical areas of gingival swelling,infection,and gum color and check the formation of fistula.(2)The alveolar bone healing situation of digital root tip(or surface fault)and a continuous shadow of the cave.(3)Whether it is suitable for removable denture,fixed bridge or implant repair treatment,evaluation of repair effect.(4)SPSS 20.0 statistical software package for data processing,X2 test for counting data and the significance of using P less than 0.05 as a difference.Results:In this study,15 cases of CGF group after maxillary sinus perforation surgery are no follow-up symptoms,infection and fistula.After 3 months,the normal gingival has a normal color,the new bone formated,alveolar fossa healed,surgery and there is no discomfort and inflammation in maxillary sinus.Nobody needs the second surgery for the denture repair treatment after 3 to 6 months.Three patients of 15 cases which are flap transfer group after maxillary sinus perforation repair follow-up showed swelling and discomfort and one patient had postoperative infection.After three months,15 patients had normal gingival color,the new bone formation of sinus floor,alveolar fusion,and there is no discomfort and inflammation in surgery area and maxillary sinus.After 3 to 6 months of the surgery,two patients need vestibular ditch surgery to repair the treatment of denture.It cannot be considered that there are differences between CGF group and flap transfer group in the postoperative swelling,infection,fistula and postoperative secondary surgery,etc.(P>0.05).Conclusion:There is no significant difference between the effects of CGF on the maxillary sinus perforation repair effect compared with the traditional flap transfer technique.Both can help to form a normal soft and hard tissue after surgery.However,the use of CGF to repair the maxillary sinus perforation can reduce postoperative adverse reactions.It has good effects in clinical application and is worthy of carrying out the application.
