Analysis the maxillary molar area maxillary sinus palatal side plate thickness and the bone mineral density of adult by CBCT in order to provide the necessary data for clinical treatment.To observe the clinical effect of maxillary sinus floor augmentation by bone condensing technique in an oblique direction and simultaneous dental implantation without grafting to test weather the surgical method is feasible and provide reference for clinical treatment.MethodsPart ?:According to the exclusion and inclusion criteria,analysis the 175 data that chosen from the people that with CBCT scan in the 105 th Hospital of PLA Center for oral from January 2015 to June 2016.Based on the comparison of the 700 measurements,the changes of the thickness of the palatal bone,the density of alveolar process and the density of the palatal bone wall of maxillary sinus were analyzed.SPSS17 software was used to analyze the data,and the results were compared with the two independent samples t-test,P<0.05 was considered statistical significant.Part ?: Selected 24 patients that undergoing dental implant in the Department of oral implant center of 105 th Hospital of PLA from January 2014 to December 2015.Informed consent was obtained before operation,and the operation methods and possible complications were informed.A total of 37 implants were implanted in maxillary molar region by the alveolar ridge approach to elevate the maxillary sinusfloor.Use single crown to complete the final restoration after six months and measure the height of new bone formation around the implant root by CBCT imaging.Statistical implant success rate of 6 months,and 12 months.ResultPart ?:(1)The following results were obtained by statistical analysis of 175 eligible imaging data: There were 93 males and 82 females with a minimum age of 26 and a maximum age of 96(mean age,57).A total of 700 measurement sites,147 first molar missing,PBT value was 4.438 + 2.377mm;203 first molars were retained,PBT value was 4.961 + 2.143mm;163 of the second molars were missing,PBT value was 3.163 +1.891mm;187 of the second molars retained,PBT value was 3.373 + 1.705 mm.The maximum PBT value of the first molar missing site was 10.48 mm and the minimum value was 0.81mm;The maximum PBT value of the second molar missing site was9.21 mm,and the minimum value was 0.58mm;The maximum PBT value of the second molar retention site was 8.64 mm,and the minimum value was 0.81 mm.(2)A total of129 bone mineral density measurement sites,the density of alveolar process was 372.78+ 207.49 HU,and the density of palatal bone wall of maxillary sinus was + 236.60 HU.Part ?: 6 months follow-up,37 implants were successfully completed single crown restoration.12 months follow-up,37 implants can function properly.One year success rate was 100%.The mean alveolar bone height in the implant area was(6.7±0.8)mm,and the height of the maxillary sinus floor elevation was(4.2 ± 0.6)mm.The mean height of osteogenesis was 2.1mm at 6 months and was 2.3mm at 12 months.Conclusion(1)There was significant difference in the maxillary palatal bone thickness between maxillary first molar sites maxillary palatal bone thickness and maxillary second molarsites,and the first molar sites of maxillary sinus palatal side wall thickness is larger than second molar sites.(2)The thickness of the palatal bone thickness of the maxillary first was larger in maxillary first molar site than maxillary first molar missing site,and the difference was statistically significant;there was no significant difference in the thickness of the palatal bone between maxillary second molar site and maxillary second molar missing site.(3)There were significant differences in bone density between alveolar process and palatal wall of maxillary sinus.(4)The bone mineral density of maxillary molar area was significantly reduced after55 years old,and the bone mineral density was higher in males than in females.(5)It is a feasible method to elevate maxillary sinus floor by extruding the cancellous bone of maxillary sinus palatal side wall in an oblique direction and the autogenous bone of maxillary sinus palatal side wall has the ability to promote new bone formation.
