Objective: The purpose of the study is through the analysis of the intestinal barrier function,inflammatory factors,liver function,renal function,nutritional statue between the control group and experimental group,to explore the clinical efficacy of complex probiotics for critically ill patients with Antibiotic-associated diarrhea(AAD).Methods 120 Critically ill patients with AAD in intensive care unit room from August 2015 to December 2016 at people's hospital of Xinjiang Uygur Autonomous Region were selected and divided into experimental group and control group at random.Two groups of patients both were received nutritional therapy based on basic therapy.In terms of nutritional therapy,the control group received conventional nutrient treatment while experimental group received nutrient solution with complex probiotics based on routine nutrient treatment.The patients in observation group were additionally given complex probiotics(30g/d)by oral administration for 6 days.To observe the changes of liver function including Alanine aminotransferase(ALT)and Aspartate transaminase(AST),renal function including test of serum creatinine(Cr)and blood urea nitrogen(BUN),asses of nutritional statue including test of serum albumin(Alb)and serum total protein(TP),inflammatory marks like C-reactive protein(CRP)? interleukin 6(IL-6)?procalcitonin(PCT),The intestinal barrier function including test of Diamine oxidase(DAO)?D-Lactic acid(D-LC)?Bacterial endotoxin(BT)between two groups,to evaluate the clinical efficacy and safety.Results Two kinds of nutritional therapy both are having great clinical effect on treatment of AAD,The levels of DAO?D-LC and BT in both groups decreased after treatment,which was more significant in experimental group than in control group after 6 days of therapy(P<0.05).After treatment,in the combined effects of a complex probiotics,the IL-6?CRP and PCT of experimental group were significantly slower than of themselves before study and of control group after treatment of 6 days(P<0.05);Compared with control group,after 6 days continuous treatment,the liver function and renal function of patients in experimental group was significantly improved than that of control group,the difference has statically significance(P<0.05).After 6 days therapy,higher level of Alb was observed in patients from experimental group when compared with control group(P<0.05),There was no significant difference in the TP level between two groups.The total clinical effective rate in experimental group was 94.7%,which was obviously higher than that in control group(77.3%)with statistical significance(Z=2.437,P<0.05).Conclusion The enteral nutrition addition to complex probiotics could improve the intestinal dysfunction of patients with AAD,protect intestinal barrier function,restore the intestinal microecological balance,reducing inflammatory response,to improve nutritional statues,repairing the liver and renal function,enhance the clinical efficacy,and significantly improve the prognosis of critical patients,which is worthy of wide use.
