Aim:within the globe,The number of deaths from infectious diseases accounts for about 30% of the total human deaths,And elderly patients due to autoimmune function decline,multiple organ dysfunction,a variety of chronic basic diseases,more susceptible to infection,The incidence was significantly higher than the young.In infectious diseases,especially respiratory tract infections has been significantly,And easily lead to a variety of adverse events,serious death.Therefore,how to prevent and control the occurrence and development of infectious diseases,improve patient prognosis and reduce mortality has become the top priority of clinical work.Clinical work found that the current elderly patients admitted to hospital due to infectious diseases are accompanied by malnutrition and immunocompromised,And malnutrition,low immune function can extend the prognosis of infected patients or even increase the original infection,such a vicious cycle,resulting in patients with prolonged healing,the body is getting worse.So for infectious patients such as early assessment,screening for its existence of malnutrition and immune dysfunction,and to give active intervention,the prognosis of patients and quality of life is extremely important.Due to infectious diseases in the highest proportion of pulmonary infection,Therefore,this study specifically investigated the immune function levels and nutritional status of patients with pulmonary infection.There is currently a survey on the nutritional level of infected patients,But for the infected patients at the same time nutritional status and immune rating of the assessment study is still limited,Especially in elderly patients with less research,This study aims to assess the nutritional and immune function of elderly patients with pulmonary infection,To investigate the intervention of malnutrition and immune dysfunction in elderly patients with pulmonary infection,And to provide the basis for the optimization of nutrition and immune support in elderly patients with pulmonary infection.Methods:140 elderly patients with pulmonary infection were selected from the cadre ward of the Hospital of Jilin University in August 2016-February 2017,60 years of age or older(mean age 84.05±7.46 years),According to the age of the patients were divided into three groups,group A(60-74 years),group B(75-89 years)and group C(?90 years),Screening and assessment of hospitalized patients,nutritional risks and immune function,And it was analyzed and compared BMI,cholesterol,LDL,uric acid,creatinine,urea nitrogen,albumin,prealbumin,hemoglobin,retinol protein,Lymphocyte count,While the analysis of pulmonary infection in the presence of malnutrition and immunocompromised elderly patients with clinical intervention ratio.Data analysis results using statistical software SPSS22.0 statistical analysis,A value of P<0.05 was considered statistically significant.Results:In this study,140 elderly patients were enrolled in nutrition screening,15 cases in group A,99 cases in group B,26 cases in group C,The incidence of total malnutrition was 68.6%(96/140),The overall incidence of low immunization was 80.7%(113/140),The incidence of total malnutrition combined with immunocompromised was 58.6%(82/140).Which different age groups of malnutrition and poor immune function of the proportion of different,In the A group,the incidence of malnutrition,the incidence of immunocompromised factors and the incidence of malnutrition combined with immune dysfunction were 40.0%,53.3% and 26.7%;In the B group followed by 66.7%,79.8% and 54.5%;In C followed by 92.3%,100% and 92.3%.The incidence of malnutrition in different age groups was significantly different(P<0.05).The incidence of malnutrition in the B group was significantly higher than that in the A group(P<0.05),The group C was significantly higher than group A(P<0.01)and group B(P<0.05).The incidence of immunocompromised in different age groups was significantly different(P<0.05).The incidence of immunocompromised group B was significantly higher than that of group A(P<0.05),in the C group was significantly higher than that in the A group(P<0.01)and group B(P<0.05).In addition,the incidence of malnutrition combined with immunocompromised in different age groups was significantly different(P<0.001).The incidence of malnutrition combined with immunocompromised in group B was significantly higher than that in group A(P<0.05),in group C was significantly higher than that in group A(P<0.001)and group B(P<0.01).In addition,the linear correlation between MNA-SF score and albumin,prealbumin,cholesterol LDL and hemoglobin was significant(P<0.05).There were significant differences in albumin,LDL,hemoglobin and body mass index(BMI)among different age groups(P<0.05).The albumin was significantly higher in the A group than in the B(P=0.014,P 0.05)group and the C group(P= 0.039,P<0.05);LDL was significantly higher in the A group than in the B(P<0.001)year group and the C group(P=0.002,P<0.01);The hemoglobin was significantly higher in the A group than in the B group(P=0.009,P<0.01)and in the C group(P<0.001),in the B group were significantly higher than those in the C group(P= 0.044,P<0.05).The level of BMI in the C group was significantly lower than that the A group(P= 0.001,P<0.01)and the B group(P= 0.012,P<0.05),in the B group was significantly lower than that in the A group(P= 0.038,P<0.05),With age,the BMI level decreased.Which found that patients with pulmonary infection combined with malnutrition group given the proportion of patients treated with nutritional support was 38.5%;While the proportion of immunocompromised patients was only 24.8% for immunocompromised patients;Malnutrition combined with low immune function at the same time to give nutrition and immune support the total treatment rate was only 14.6%.Conclusions:(1)Lung infection in elderly patients,the incidence of malnutrition and poor immune function is high,and with age,more prone to malnutrition combined with immune dysfunction.(2)Pulmonary infection in the presence of malnutrition and immunocompromised elderly patients,clinical work to give nutrition and immunization support the proportion of low.(3)Albumin,prealbumin,cholesterol,LDL and the level of hemoglobin are related to the nutritional status of the body,Where albumin LDL and hemoglobin levels are associated with age.
