OBJECTIVE:To investigate the current situation of nutrition support medication in adult hospitalized patients and to developclinical pharmacy practice on nutrition support.METHODS:1) Use retrospective entire medical orders analysis, investigate the current situation of nutrition support medication in adult hospitalized patients.2) Rewiew clinical pharmacy practice on nutrition support and develop nutrition support pharmacist practice and education mode, and Drug Information Service standard procedure.3) Prospective pharmaceutical interventions carried by clinical pharmacist on total parenteral nutrition prescription.RESULTS:lnvolved PN and ENordersof eight hospitals from 2011 to 2014, and analyzed the consumption of PN and EN, nutrition support patients proportion, constituent ratio of different PN administrations, PN and EN population and nutrition support combination form; Develop nutrition support pharmacist practice and education mode, and Drug Information Service standard procedure; A total of 160 interventions and 44 times error prevention(154 of 6454 prescriptions) were carried out during the study period. The overall prescription intervention rate and acceptance rate are 2.39% and 98.70% respectively. There were 148 patients recruited. There were significant differences in the liver function index(Tbil and Dbil) (△Tbil custom-prepared parenteral nutrition group:-0.77±4.77, standardized parenteral nutrition group:6.74±12.18, p-0,013; △Dbil custom-prepared parenteral nutrition group:0.41±1.93,standardized parenteral nutrition group:5.09±7.78, p=0.001)CONCLUSION:The use of nutrition support medication in 8 tertiary hospitals became more appropriate, but still inappropriate on MCB and PN/EN ratio compare to occident; Nutrition support pharmacist practice and education mode, Drug Information Service standard procedure, Pharmaceutical interventions on TPN and error prevention can promote safe, rational, effective and economical outcome on nutrition support patients. Custom-prepared parenteral nutrition has less effects on bilirubin compare to standardized parenteral nutrition on gastric cancer gastrectomy.
