Objective:The research is to investigate the epidemiology,clinical manifestation,auxiliary examination and other characteristics of brucellosis,in order to provide data for the correct diagnosis of clinicians.Method:512 diagnosed brucellosis medical records of hospitalized patients were collected in the First Affiliated Hospital of Jilin University from January 1,2009 to June 31,which included general information,epidemiological contact history,clinical manifestations(symptoms and signs),complications,diagnosis results(clinical diagnosis,laboratory examination etc.).Epidata3.0 was used to establish a database,and SPSS21.0 was used for data statistics and analysis.The comparison of enumeration data was made by chi square test.Result:1.512 cases of brucellosis patients were predominantly male,the sex ratio was 2.51:1.Age mainly concentrated in 41-60 years old,accounting for 45.51%.The Han nationality population had the greatest number(92.97%).The major occupation were farmers and herdsmen,accounting for 75.78%.Most of the patients were rural population(71.09%).2.In the 512 cases of brucellosis patients,419 cases(81.84%)were with clear history of exposure,93 cases(18.16%)with not quite clear contact history.The main contact animals were cattle and sheep,and the most was sheep.The main contact mode was feeding or grazing.3.The clinical stage was mainly in acute stage(93.75%),and the proportion of subacute and chronic patients was 4.10% and 2.15%,respectively.4.The incidence of fever was the highest,and the incidence was 94.14%.Followed by joint pain,chills,splenomegaly,incidence rate were 35.74%,32.62% and 26.17%.The patients with rash symptoms were few.83.79% of the patients with brucellosis have other complications,and hypoproteinemia,anemia and osteoarthritis were the main complication.5.Clinical auxiliary examination: blood routine examination: 78 cases of white blood cells increased,decreased in 86 cases;85 cases of neutrophil percentage increased,decreased in 52 cases;97 cases of lymphocyte percentage decreased;65 cases of percentage of mononuclear cells increased;230 cases of red blood cells decreased;211 cases of hemoglobin decreased;38 cases of platelets increased,decreased in 138 cases.Liver function: 0 cases of albumin increased,decreased in 430 cases;15 cases of globulin decreased;elevated alanine aminotransferase in 240 cases;aspartate aminotransferase increased in 272 cases;270 cases of GGT increased;175 cases of alkaline phosphatase increased.6.ESR and C-reactive protein: Most of the 512 cases with elevated ESR,accounting for 53.91%.Most of the C reactive protein was elevated,and the constituent ratio was 70.90%.7.Results of 397 cases,we found 224 positive blood cultures,the positive rates was 56.42%.The difference of clinical features and the incidence of joint pain,splenomegaly,liver enlargement,hypoalbuminemia and anemia between positive blood culture patients and negative patients showed statistically significance,P< 0.05.The incidence of decreased white blood cellcounts,increased Neutrophil percentage,the hemoglobin content decreased and decreased plateletcount in positive blood culture patients was higher than these in negative patients,P < 0.05.The incidence of the reduction of urinary albumin,increased aspartate aminotransferase and increased alkaline phosphatase in positive blood culture patients was higher than these in negative patients,P<0.05.The difference of erythrocyte sedimentation and C-reactive protein between two groups showed no statistically significance,P>0.05.Conclusion:1.Brucellosis is given priority to with male patients,the farmers and herdsmen were the main infected occupation.2.81.84% patients were with a history of epidemiological contact,Cattle and sheep is the main contacted animals,and sheep is the most contacted animal(74.61%).3.Brucella patients with the acute phase is given priority to(93.75%).The main clinical manifestations were fever(94.14%),joint pain(35.74%),chill(32.62%)and splenomegaly(26.17%).The main complications were hypoproteinemia(37.30%),osteoarthritis(37.30%)and anemia(34.18%).4.53.91% patients were with elevated ESR and 70.90% were with elevated CRP.Common liver function of the patients with brucellosis was obviously damaged,and widespread poor nutritional status.5.The incidence of joint pain,splenomegaly,liver enlargement,hypoalbuminemia and anemia in positive blood culture patients were higher than these in negative patients.Liver dysfunction status in patients with positive blood cultures was more severe than that in patients with negative blood cultures.
