Influence Of Follow-up Intervention On Social Support And Medication Compliance For HIV/AIDS Patients

Objective The purpose of this study was to assess the levels of social support and the adherence to highly active antiretroviral therapy(HARRT)for HIV/AIDS patients in Henan Province,to evaluate the effects of follow-up intervention on HIV/AIDS patients.It could provide valuable information for improving social support and the adherence to HARRT in the future.Methods Stratified random sampling method was used to include HIV/AIDS patients from the five demonstration city in Henan province.Baseline and follow-up survey were conducted among participates to analyze the social support and the adherence to HARRT using social support rating scale and antiretroviral medication self-report questionnaires.And 59 respondents were interviewed through the method of semi-structured qualitative interview.Results A total of 911 and 860 HIV/AIDS patients were included by the baseline and follow-up survey,among each,798 and 795 participants were accepted HARRT.Women account for a large proportion,the baseline survey of women were 454 cases,accounting for 56.9%,the follow-up survey of women were 428 cases,accounting for 53.8%.Statistical analysis showed that the baseline and follow-up survey in gender,age,nationality,cultural degree,marital status,profession and so on had no statistical significance(P>0.05),in addition to personal monthly income.It showed that the baseline and follow-up survey had higher comparability.The total score of social support and its all dimensions in the following-up survey were higher than those of the baseline survey(P <0.05).The mean and standard deviation of subjective social support was 24.91±5.10,the mean and standard deviation of objective social support was 8.76±2.38,the mean and standard deviation of the utility of social support was 6.71±2.18 in the follow-up survey,those of the baseline survey were 24.13±5.09?8.23±2.76?6.29±2.26.567 cases of patients were found have better medication compliance,and the corresponding number was 675 cases of patients after the follow-up intervention,and the significant statistical differences were found(P <0.01).The patients who had the higher social support could had better medication compliance.The further analysis showed that the total score of social support and its all dimensions on HIV/AIDS patients were positively correlated with adherence to HARRT before and after the follow-up intervention(P<0.05).There had no ststistically significance in the social support about the patients with illiteracy,other occupations,health status and high income,other factors had statistically significance(P<0.05).Multiple logistic analysis showed that the main factors affected the compliance might include:subjective social support(P=0.002,OR=1.075).The qualitative results about the 59 patients showed that most of the patients thought that the follow-up intervention could help them to solve problems in time.However,a few doctors were not enthusiastic about the patient,and even discriminate against the patients.Conclusion Follow-up intervention could significantly improve the social support and the adherence to HARRT.When we urged the patients to take medication on time,we need to establish social care support mechanism relying on the family,in order to advance the level of social support.The higher the score of social support,the better the medication compliance,the social support of the HIV/AIDS patients played an important role about the adherence to HARRT.We should took measure to follow to the patients whose social support and medication compliance had no obvious effect before and after the follow-up intervention in different ways.
