Objective:Good medication compliance is a standard rational drug use, it can enhance the effect of drugs, and promote the rehabilitation of patients, when the treatment regimen is effective, at the same time, it can also save health care resources and reduce medical costs. Medication non-compliance has become a common phenomenon at present. This study wants to understand the status situation of the patient medication adherence behavior, medication knowledge, self-efficacy and perceived social support, explore and analyze the main factors affecting patient medication compliance, and put forward intervention strategies to increase medication adherence from individual, groups, social perspectives, through the investigation and analysis of patient medication compliance and influencing factors.Methods:Investigate the medication compliance behavior and its predictors among 800 patients who visited hospital or primary care health institutions for treatment using stratified random sampling method. According to Hangzhou city, suburban population and the proportion of clinic visits to hospitals or primary health care institutions, patients were divided into the source of the city hospitals, community health service centers, rural hospitals, rural primary health centers, A total of 800 questionnaires were sent out,784 valid questionnaires. Descriptive analysis presented the status situation of patient medication adherence behavior, medication knowledge, self-efficacy and social support. Use analysis of variance, t test, chi-square test to analyze the differences in different characteristics of the patient medication knowledge, self-efficacy, perceived social support and medication adherence behavior. Explore the main factors affecting medication adherence behavior with multiple linear regression and binary logistic regression analysis, so as to propose suggests of increasing the patient medication compliance.Results:1. In this study, medication adherence scale score (1.66±1.37) points,106 cases have good compliance, while 678 cases have poor compliance, compliance rate was 13.5%. The results show that there were 480 cases patients who ever forgot to take drugs,342 cases patients who did not pay attention to medication,520 cases patients who stopped the medication after symptoms turned better, and 495 cases patients stopped the medication after the symptoms turned worse, among the four common reasons affecting medication adherence.2. In surveyed patients, patient medication knowledge score (19.0±5.0) points. 207 patients with a high level of medication knowledge, accounting for 26.4%, moderate level of 397 patients, accounting for 50.6%, a low level of 179 patients, accounting for 22.8%. Patient medication knowledge and medication compliance is positively correlated. Education level, the degree of attention to the disease, the number of drugs, self-efficacy, perceived social support are the main factors that influence medication knowledge level of the patient;3. In surveyed patients, self-efficacy scale score (25.21±5.77) points. Patient self-efficacy is positively correlated with adherence to medication. Family income, the level of medication knowledge, perceived social support are the main factors of patient self-efficacy;4. In surveyed patients, perceived social support score (44.27±9.21) points. Perceived social support and patient medication compliance is positively correlated. Gender, living conditions, health insurance type, household income, the degree of attention to the disease, self-efficacy, medication knowledge become the main factors of patient perceived social support;5. In univariate analysis of patient medication compliance behavior, the results show that the differences of medication compliance of patients in different age, education, occupation type, family income, chronic diseases, the impact of disease on life, medication knowledge, self-efficacy, perceived social support were statistically significant.6. In univariate analysis of patient medication compliance behavior, the results show that the differences of medication compliance of patients with chronic disease in different age, education, living conditions, occupation type, type of health insurance, family income, the sick time, the degree of focus on disease, perceived social support were statistically significant.7. In multivariate analysis of patient medication compliance behavior, the results show that medication knowledge, age, education level were the main factors in patient medication compliance, where medication knowledge effected the most, followed by the age and educational level.Conclusion:Patients had poor medication adherence, age, education, occupation type, family income, chronic diseases, the impact of disease on life, medication knowledge, self-efficacy, perceived social support have an impact on patient medication compliance behavior, which, age, education level and medication knowledge are major factors. Age, education, living conditions, occupation type, type of health insurance, family income, the sick time, the degree of focus on disease, perceived social support are factors in patients medication compliance with chronic disease. Therefore, in order to improve patient compliance, firstly we must promote the doctor-patient communication, so as to create a good medical environment; secondly, strengthen health education to improve patients’ medication knowledge and self-efficacy; Last, establish a good family and social support systems and create a social environment caring for the patients.
