Objective:To analyze the correlation between infantile eczema and serum food sIgG?sIgE,and provide a theoretical reference about diet for the clinical treatment of infants with eczema in Changchun area by detecting serum food sIgG and sIgE both in infants with eczema and healthy controls.Methods:This study set up two groups: 156 infants diagnosed with eczema form department of Dermatology of the First Hospital of JILIN University,meeting inclusion and exclusion criteria,were recruited as eczema group,and 23 cases of healthy infants were randomly selected as control group.The detection of food sIgG and sIgE were implemented for every infants in both two groups.The food sIgG were detected by enzyme-linked immunosorbent assay(ELISA),including 14 kinds of foods: beef,milk,egg white / egg yolk,chicken,pork,shrimp,crab,codfish,rice,corn,wheat,tomato,soybean and mushroom.The food sIgE were detected by Western blot analysis,including 10 kinds of foods: egg,milk,shellfish,shrimp,crab,beef,cashew,pineapple,mango and amaranth.Results:1.The positive rate of sIgG was 83.97% in eczema group and 13.04% in 23 cases of control group.The positive rate of eczema group was significantly higher than that of the control group,the difference was statistically significant(c2=53.59,P< 0.001).2.The positive rate of sIgG for 14 kinds of food in eczema group was listed as follows: milk(63.46%),egg white / egg yolk(29.49%),tomato(23.72%),wheat(18.59%),soybean(10.26%),rice(10.90%),corn(4.77%),beef(4.49%),codfish(4.49%),pork(1.92%),chicken(0.64%)and crab(0.64%),shrimp was not detected.Among the131 patients with positive reactions,the number of sIgG-positive foods varied from 1to 7,with 1 to 2 species more common.The proportion of sIgG positive only for one kind of foods was 45.04%(59/131);and the composition ratio of sIgG positive for two kinds of food was 26.72%(35/131).Highly sensitive food was followed by milk(34.62%)> egg white / egg yolk(10.26%)> wheat(5.77%),mediumly sensitive food was followed by milk(19.23%)> egg white /egg yolk(10.26%)> Tomatoes(8.97%),mildly sensitive food was followed by tomatoes(10.90%)> milk(9.62%)>egg white / egg yolk(8.97%).3.Among the 156 cases of eczema group,53 cases of children were positive for sIgE,the positive rate was 33.97%,and for 23 cases of control group,only 2 cases of food sIgE positive,the positive rate was 8.70%.The difference between the two groups was significant and statistically significant(c2 =27.17,P <0.001).4.Food sIgE positive rate test results in eczema group: milk(23.72%)> egg white(14.74%)> cashew(5.77%)> mango(1.92%)= pineapple(1.92%)> beef(1.28%)>shrimp(0.64%)= crab(0.64%),shellfish and amaranth were not detected.There were1 to 5 species of sIgE positive in a variety of foods.Of the 53 patients with sIgE positive,67.92%(36/53)of the children had only one kind of food sIgE positive.According to the content of food sIgE detection level,+1 grade food was milk and eggs,each just accounted for 2 cases;+2 grade food allergens were mainly milk(22.44%),egg white(9.62%)and cashew nuts(5.13%);for +3 grade there were 7cases,of which 6 cases were all allergic to egg white,and the left one allergen was cashew nuts.5.The positive rates of sIgG and sIgE in eczema group were 83.97% and 33.97%,respectively.The positive rate was statistically significant(c2 = 69.14,P <0.05)by Mc Nemar test.There was no correlation between the two methods(c2 = 2.74,P >0.05).The results showed that there were significant differences in the positive rates of milk and eggs between the two methods(p <0.05),and the other three differences were not statistically significant(p>0.05).Conclusion:1.Infantile eczema is related with both food sIgG and sIgE,sIgG and sIgE are involved in the pathogenesis of eczema.2.In Changchun area,for more than 80% infantile eczema patients,the food sIgGtest results are positive,that is,there is food intolerance.Mainly intolerant food is milk,egg white /egg yolk,tomato and wheat.3.Intolerant food involves a variety of species,with 1 to 2 species more common.Medium and severe sensitive foods are mainly milk and egg white / egg yolk.4.Milk and chicken protein is the leading cause of increased sIgE,inducing food allergy for infants with eczema in Changchun area.And all kinds of allergic food is mainly to a kind of main.5.The main food allergens which leading to increased sIgG and sIgE for infants with eczema in Changchun area are milk and eggs,but the detection rate of the two methods was different,and the results were not correlated.In clinical application,they can not substitute for each otherr.
