| Objective: To compare the clinical data of paitients with proximal humeral fractures treated with anatomical locking plate and Acumed intramedullary nail fixation.Methods: Follow up patients who suffered two part fractures of the proximal humerus in the department of orthopedic traumatology of our hospital from July 2014 to October 2016.68 cases were grouped according to the different surgical methods,40 patients with open reduction and anatomical plate internal fixation were divided into A group.28 patients with Acumed intramedullary nail fixation were divided into B group.Patients were followed up for 6 months at least,the observation indicators include the incision length,bleeding volume,operation time,fracture healing time,complications,shoulder joint activity appeared at the last time follow-up,postoperative shoulder function scores,and the results were statistically analyzed.Results: the operation time,blood loss,incision length,fracture healing time of A group(Lookng plate group)were higher than those of B group(intramedullary nail group),and the difference between the two groups was statistically significant(P<0.05).There was no significant difference between the A group(Lookng plate group)and B group(intramedullary nail group)in the activity of shoulder and shoulder function score at the last follow-up(P>0.05).There were 1 patient in the A group(Lookng plate group)with limited shoulder joint activity and 1 paitient delayed union and healing at the end of seventh months postoperation.There were1 cases of shoulder pain in B group(intramedullary nail group).Conclusion: 1.For two parts of the the proximal humerus fracture the intramedullary nail and locking plate fixation can make long-term joint function recovery,can achieve good clinical curative effect.2..For two parts of the the proximal humerus fracture,in the treatment of intramedullary nail fixation,the operation time is shorter,the amount of bleeding is less,the incision length is small,the fracture healing time is short,intramedullary nail has more advantagers than the locking plate. |
