ObjectiveTo evaluate the incidence of sensorineural hearing loss in patients with vitiligo, study the effect of gender, age, onset age, course of disease, types, family history on hearing in patients with vitiligo, and investigate the relationship between hearing changes and autoimmunity in patients with vitiligo.Methods1.Study objects:This study analyzed 40 cases of vitiligo who go to The First Affiliated Hospital of Zhengzhou University from January 2016 to January 2017.35 cases were matched with age and gender in healthy volunteers.Patients who had history or evidence of audiological diseases;familial hearing loss;chronic noise exposure;head trauma;metabolic, neurological, vascular,or autoimmune disease;any systemic diseases such as diabetes or hypertension;age >46years;or use of oral autotoxic or corticosteroids were excluded from the study.Survey indicators include name, gender, age, types, address, telephone number, age, family history, course of disease, pure tone audiometry(PTA),auditory brainstem response(ABR),free triiodothyronine(FT3),free thyroxine(FT4)and thyroid stimulating hormone(TSH),anti thyroid peroxidase antibody(TPOAB)and anti thyroglobulin antibody(TGAB),thyrotropin receptor antibody(TRAB).2.Statistical analysis: Data were analyzed using SPSS 22.0 software. Results were expressed as mean (x|-)± s for quantitative variables,t text was used for comparison between groups;Spearman correlation coefficient to describe correlation between two groups of variables; Results were expressed as percentage for categorical variables,?~2 test was used for comparison between groups.P values less than 0.05 were accepted as the significance level.Results1.A total of 40 cases of patients with vitiligo, which is associated with sensorineural hearing loss in 16 cases(40%),a total of 35 cases of the control group, which is associated with sensorineural hearing loss in 2 cases(5.71%),the prevalence rate of vitiligo patients with sensorineural hearing loss is higher than the control group, and the difference was statistically significant(P<0.05).Vitiligo patients with sensorineural hearing loss are mostly high frequency.2.Auditory brainstem response on the right side of III value and I-III peak interval of vitiligo group were longer than the control group, and the difference was statistically significant(P<0.05),there was no significant difference in the other latency and peak interval values(P>0.05).3.TPOAB of the vitiligo group was higher than that in control group, and the difference was statistically significant(P<0.05),there is no significant difference in FT3?FT4?TSH?TGAB and TRAB between two groups(P>0.05).4.The TPOAB value of the vitiligo group was positively correlated with the right III latency value and the right I-III peak interval(R=0.402,0.403,P<0.05),and was negatively correlated with the III-V peak interval(R=--0.346,P<0.05).TGAB value was negatively correlated with left III latency(R=-0.366,P<0.05),and the difference was statistically significant(P<0.05).The value of TRAB was positively correlated with right I,V latency and right III-V peak interval(R=0.485,0.360,0.384,P<0.05).FT3 was negatively correlated with left I,V latency, left I-V peak interval and left III-V negative correlation, the correlation coefficients(R=-0.358,-0.450,-0.317,-0.376,P<0.05).TSH was negatively correlated with the right III-V peak interval and left I latency(R=-0.344,-0.439,P<0.05).5.The incidence of generalized vitiligo with sensorineural hearing loss was higher than the one of segmental vitiligo with sensorineural hearing loss, and the difference is statistically significant(?~2=4.20,P<0.05).There was no statistically significant difference between other types(P>0.05).6.There was no significant difference in gender, age, age of onset, course of disease and family history of vitiligo with sensorineural hearing loss.Conclusions1.Vitiligo can be combined with sensorineural hearing loss, and the prevalence rate of sensorineural hearing loss in vitiligo patients is higher than that in healthy control group, vitiligo associated with sensorineural hearing loss is mostly high frequency,we should screen for sensorineural hearing loss in patients with vitiligo in clinic by evaluating the hearing loss in patients with vitiligo;2.Vitiligo patients with sensorineural hearing loss has some correlation with thyroid function and autoantibodies, which may be related to autoimmune pathogenesis of vitiligo;3.The incidence of generalized vitiligo with sensorineural hearing loss was higher than the one of segmental vitiligo with sensorineural hearing loss;4.There was no significant difference in gender, age, age of onset, course of disease and family history of vitiligo with sensorineural hearing loss;...
