Analysis On The Effect Of Comprehensive Reform Of Public Hospitals At County Level In Zhengzhou

ObjectiveBy studying the operating status,resources and service utilization of 11 county-level public hospitals in 5 counties(cities)in Zhengzhou from 2010 to 2015,to evaluate the effect of comprehensive reform of public hospitals at county level in Zhengzhou City,find out the existing problems,and put forward some suggestions for the problems.MethodsThrough the questionnaire survey and field investigation,we collected the specific operation data of 11 hospitals of Zhengzhou in the county public hospital comprehensive reform and established the database,to analyse the operation changes of the hospitals from 2010 to 2015,and taked 2013 as a node for comparative analysis of these hospitals before and after reform.Results1 1.Basic situation of hospital reformThe on-the-job number of 11 county-level public hospitals in Zhengzhou increased from 4403 in 2010 to 7291 in 2015 with an increase of 65.59%;The number of designated beds increased from 2677 in 2010 to 5744 in 2015 with an increase of 114.57%;Revenue increased from 633 million 290 thousand yuan in 2010 to 1 billion 787 million and 740 thousand yuan in 2015 with an increase of 182.29%;Financial subsidy income increased from 34 million 530 thousand yuan in 2010 to 206 million 560 thousand yuan in 2015 with an increase of 498.2%.After the reform,the public health resources and technical level of the county public hospitals in Zhengzhou have been greatly improved,the debt situation has been reversed,the income and expenditure have been balanced,and the county public hospitals have been developed by leaps and bounds.2.Service utilization of hospital reformThe Total number of clinics of 11 county-level public hospitals in Zhengzhou increased from 1871330 in 2010 to a total of 3843765 in 2015,an inceares of 104.4%.The number of Door(emergency)number of patients increased from 1708067 in 2010 to 3582858 in 2015,an increase of 109.76%.The mumber of inpatients increased from 163263 in 2010 to 260907 in 2015,an increase of 59.81%.After the reform,the service ability and utilization efficiency of the county public hospitals in Zhengzhou have been promoted,and the accessibility,availability and fairness of health services in the county have been improved.3.The cognition of medical staff and patients to hospitalThe satisfaction degree of the medical staff to the reform measures,the hospital running condition,the hospital administrative efficiency,the hospital management system and so on are all above 90%,and the satisfaction to the income is low,is 67.7%.The satisfaction degree of the patients to the hospital environment,the procedure,the service attitude of the medical staff,the cost of treatment were all above 90%.Medical staff and patients are satisfied with the current situation of the hospitals,but the demands of medical staff on their own income needs to be further satisfied.ConclusionThe achievements of the reform in public hospitals at county level in Zhengzhou is remarkable,while the exsisting problems are as follows: Grading system is not perfect,some patients in the county without referral to large municipal and provincial hospitals,standardized and orderly medical order has not yet formed.The medical service price is low,the personnel system and the distribution system are not perfect,the medical staff's income and the treatment do not match with the work intensity,has affected the work enthusiasm of medical staff.
