Background and Objective To investigate the validity of quantitative handwriting analysis parameter for the diagnosis of Parkinson's disease,and its ralationship to PD severity.Methods A total of 45 patients with Parkinson's disease were enrolled from People's Liberation Army general hospital of China, 28 early PD (HY stage: 1-2.5) and 7 advanced stage PD (HY stage: 3 ) according to the inclusion criterai from QSBB,and 40 education?sex?age-matched health control were enrolled in the study.The name, sex, age,educational status,duration of disease,Hoehn-Yahr(HY)score,Schwab and England Activities of Daily Living Scale, MDS-UPDRS ?????score,PDQ-39,Mini-mental StateExamination(MMSE), MoCA, BMI, subtype while onset, family history, HADS were recorded, then all subjects were asked to accomplish three tasks on a digital board, kinetic parameters during handwriting (velocity?acceleration?movement time?amplitude?frequency of tremor) were recorded from laptop, compare the difference of all parameters between PD & HC and its relation to MDS-UPDRS?,HY stage?duration.Results Compared with the heath control group,the median velocity?acceleration?frequency of tremor significantly reduced (p<0.05) ; movement time?amplitude of tremor significantly increased (p<0.05 );?All parameters of bradykinesia and tremor in PD are well correlated with MDS-UDPRS?? and is positive correlated to HY score?duration.Conclusion The parameters seized from handwriting trajectory by digital tablet and electronic pen can efficiently detect prodromal to advanced PD,so it is a promising diagnosis tool for prodromal PD.Furthermore,this new method can objectively quantify nearly all stages of PD, Compared to traditionally used clinical rating scale, it is virtually quick?sensitive?simple and objective for the diagnosis and evaluation of the motor symptoms of PD.
