The Study On The Change Of Hospitalization Expenses Under The Background Of City-level Public Hospital Reform In Hefei

Objective:(1)Through the research on the basic theory of City-level Public Hospital Reform,the current situation of City-level Public Hospital Reform can be known and the knowledge of City-level Public Hospital Reform are studied.(2)According to the changes of hospitalization expenses before and after the City-level Public Hospital Reform,to understand the effect of the implementation of the relevant measures of City-level Public Hospital Reform.(3)Through the analysis of the changes in the structure of hospitalization costs before and after the City-level Public Hospital Reform,to analyzes whether the structure of hospitalization expenses is more reasonable.(4)To explore the differences among the different hospitals,and the differences among the different departments in the same hospital,whether there is a change in the cost of hospitalization.(5)On the basis of the results,we can provide related policing suggestions for the smooth progress of City-level Public Hospital Reform.Method: This paper makes a comparative study on the cost information of one year before and after the City-level Public Hospital Reform(From April 2014 to March2016)in three hospitals in Hefei,to ascertain the effect of the control measures of City-level Public Hospital Reform on the cost control of inpatients from Hefei.The cost of hospitalization is divided into six parts: the average drug fee,the average inspection fee,the average treatment fee,the average operating costs,the average material fee,the average hospitalization expenses,Using Excel2007 and SPSS 20 for statistical analysis.Results:(1)The hospitalization costs of three public hospitals in the city of Hefei have been controlled basically after the City-level Public Hospital Reform,the average drug costs are decreased in different degrees in accordance with the direction of policy control,hospital A average drug costs decreased significantly(fell 20.1%);In addition to hospital A inspection fees were decrease(10.1%),hospital B and hospital C both increase;Both the average treatment fee and the average operating costs were increased to a certain extent;the average cost of materials rise constantly,the Control effect of City-level Public Hospital Reform is not satisfactory.The overall structure of hospitalization costs tends to rationalize.(2)After the implementation of the City-level Public Hospital Reform in three hospitals,the changes in the cost of hospitalization showed differences each other.(3)There are significant differences in the cost of medical and surgical after the City-level Public Hospital Reform.Conclusion:(1)The City-level Public Hospital Reform in Anhui province has a certain effect on the hospitalization expenses in Hefei,which makes the total cost of inpatient stay at a relatively stable level.(2)The City-level Public Hospital Reform in Anhui controlled the cost of drugs in the hospital expense by abolishing Drug Mark-up,Compared with the cost of drugs before the reform,the drug cost is reducted,the proportion of drug costs also reducted.The inspection fee under Control and the cost of medical services such as operating expenses and treatment costs accounting for the proportion of hospitalization expenses have been Increase by adjusting the price of imaging examination items and raise the price of medical treatment and surgery.The structure of hospitalization expenses became more reasonable after the reform.(3)The change of hospitalization expenses in different hospitals are inequality,The effect of measures on the City-level Public Hospital Reform in different hospitals is different.Preparing implementation details on hospital will be more conducive to differentiated management.(4)The impact is different between medical diseases and surgical diseases in hospital costs after the City-level Public Hospital Reform.(5)The cost of materials and the proportion of the cost of materials in hospitalization expenses remain at a high level.Effective control of the cost of materials is an important guarantee for the rationalization of hospitalization expenses.Suggestions:(1)To strengthen drug cost control continuously and implement the policy of zero margin drug profit.(2)The fees of inspection Maintain a high level,we should explore the new ways to control the fees of hospital inspection.(3)The Different among hospitals and Department should be taken into consideration in the City-level Public Hospital Reform.(4)The cost of medical materials needs to be controlled urgently,and continue to implement the purchase with the amount of medical consumables.
