Objective: To study the serum levels of 1,25-dihydroxyvitamin D3 and eosinophils in patients with refractory allergic rhinitis(AR)and to observe the clinical efficacy of vitamin D calcium chewable tablets in the application of these patients.It can provide further theoretical basis for clinical diagnosis and treatment of adults with refractory allergic rhinitis.Methods: Sixty adult patients with refractory moderate to severe AR who were enrolled in the First Affiliated Hospital of Anhui Medical University from May 2014 to November 2015 were enrolled in this study.All patients were tested for serum levels of 1,25-dihydroxyvitamin D3 and eosinophils before treatment.The 60 patients were randomly divided into experimental group and control group and given different treatment,each group of 30 cases.At the same time serum vitamin D and eosinophils level of 60 healthy people were selected in our hospital health examination outpatient randomly as a healthy group.The patients in the control group were treated with fluticasone propionate nasal spray and levocetirizine.The experimental group was treated with vitamin D calcium chewable tablets on the basis of the control group.The visual analogue scale(VAS)was used to evaluate the overall symptoms and nasal classification symptoms before and after treatment and two weeks after treatment.Results: Skin prick test results: positive results of 60 cases in patients with allergic rhinitis allergen skin test,accounting for 100% of the total.13 cases of a single allergen sensitization,accounting for the total(21.6%).47 cases of the two and more than two allergen sensitization,accounting for the total(78.4%).Serum-specific Ig E test: allergicrhinitis patients with serum-specific Ig E positive results in 60 cases,accounting for 100% of the total.The content of serum 1,25-dihydroxyvitamin D3 in patients with allergic rhinitis was(17.46 ± 4.94)ng / ml,which was significantly lower than that in healthy subjects(21.70 ± 7.32)ng / ml.The difference of the two group was statistically significant(P <0.05).The content of serum eosinophils in allergic patients was 1.76 × 109 / L,which was significantly higher than that in healthy subjects(0.46 × 109 / L).There was significant difference between the two groups(P <0.05).There were no significant differences in the overall symptoms and nasal symptoms between the experimental group and the control group before treatment.The scores of the experimental group and the control group after 14 days of treatment compare with before treatment were significantly decreased,the difference was statistically significant.The scores of the experimental group and control group patients after 28 days of treatment compared with after 14 days were significantly decreased except runny nose symptoms.There was no significant difference in nasal congestion,nasal itching and sneezing between the experimental group and the control group after 14 days of treatment and the difference between runny nose and total symptoms was statistically significant.After 28 days of treatment,the overall symptoms and nasal symptoms of the experimental group were lower than the control group,the difference was statistically significant.Conclusion: Deficiency of serum 1,25-dihydroxyvitamin D3 and elevated eosinophils in patients with refractory allergic rhinitis are prevalent.Plus vitamin D calcium chewable tablets in the treatment of refractory allergic rhinitis can be more effective control of allergic rhinitis related symptoms.
