ObjectivesThrough to investigate effect of Buqi Tongluo method on Aβ protein、ET and NO expression after operation of cervical spondylotic to study the pathological changes of spinal cord injury in patients with cervical spondylotic myelopathy(SCI)and the intervention mechanism of traditional Chinese medicine in the treatment of cervical spinal cord injury.Methods60 cases of cervical spondylotic myelopathy patients from Shenzhen Pingle orthopedic hospital from 2016.03 to 2017.02 months were randomized into group A(control group)30 cases and group B(treatment group)30 cases.Two groups of patients were all treated by surgery,group A:routine treatment after operation;group B:give Bu Yang Huan Wu decoction two days after operation+routine treatment after operation.To detect and record A β protein、ET and NO in blood in different time points before operation、first days after operation、after operation for 1 weeks and after operation for 4 weeks;Cervical JOA score;Nerve root injury was detected by electromyography before operation and 4 weeks after operation.The changes of each index at different time before and after treatment were compared between the treatment group and the control group.Results1.general data:The treatment group(group B)compared with the control group(A group),gender、age、course of disease、the number of vertebral body、operation time and intraoperative blood loss ware not statistically significant(P>0.05).2.the plasma levels of endothelin(ET),nitric oxide(NO),Aβ protein in the two groups were not statistically significant(P>0.05).3.Biochemical indicators:The levels of plasma endothelin(ET),nitric oxide(NO),Aβ protein were compared between the treatment group and the control group on the 1 day after operation and showed no significant difference(P>0.05);Compared with the control group,plasma endothelin(ET)and nitric oxide(NO)decreased in the 1 week and 4 week after operation,and the difference was statistically significant(P<0.05);At 1 weeks after operation,there was no significant difference in Aβ protein in blood(P>0.05);At 4 weeks after operation,the levels of Aβ protein in blood plasma decreased,and the difference was statistically significant(P<0.05).4.Evaluation of clinical efficacy:The treatment group compared with the control group,JOA score was not significantly different in first days after surgery,(P>0.05);JOA score was significant difference at the 1 weeks and 4 weeks after operation(P>0.05);The treatment group compared with the control group,3 cases were positive in the treatment group and 10 cases in the control group with EMG detection of nerve root injury at 4 weeks after operation.ConclusionThe nerve injury in patients with cervical spondylotic myelopathy is a process of regeneration and repair.The TCM Buqi Tongluo method and Bu Yang Huan Wu decoction can make patients with postoperative plasma endothelin(ET)decreased;Bu Yang Huan Wu decoction can regulate the high concentration of nitric oxide(NO)in blood of patients with cervical spondylotic myelopathy before operation;Aβ protein decreased at 4 weeks after operation;Cervical JOA score increased A protein decreased at the 1 weeks and 4 weeks after operation;Bu Yang Huan Wu decoction can promote the recovery of nerve function after surgery and has the clinical efficacy of cervical spinal cord function recovery after surgery.
