| Cervical Spondylotic Myelopathy (CSM) is defined as functional disturbance of spinal cord as the result of external compression on and/or insufficient blood supply for spinal cord caused by degeneration of cervical vertebrae. CSM is the most popular style of spinal cord dysfunction for the people over 40. The amount of CSM patients is about 10 to 15 percent of cervical spondylopathy patients. CSM could severely influence the patients' life quality, even be life threatening. Principle on the treatment of CSM is carrying conservative therapy first, if ineffective, taking operative therapy. But it can't be forecasted to what degree the patients can recover after operation. Deng Jingfeng, professor of Guangdong Provincial Hospital of T.CM, adapted Chinese herb which can tonify the kidney and promote blood circulation to prevent and cure CSM acronling to trKdmenoal diagnosis character of CSM (kidney asthenia and blood stasis), and got much satisfying therapeutic effectiveness. The article carried out clinical and experimental research on the Chinese herb with effect of tonify -kidney and move-blood, discussed the relationship between endothelin (ET) and the treatment of CSM by means of Chinese herb, and founded the basis finally for the research of mechanism of treatment of CSM with Chinese herb. 1. ReviewThe article summarized the pathogenic cause, diagnosis and treatment on CSM by referring to the internal and external research accomplishment of CSM during the last 5 years. CSM belongs to die field of "XiangQiang", "WeiChu", "GuBi" according to T.C.M.. The cause is Mgue damaging neck and on the second influence liver and kidney. The pathogenis is asthenia of liver and kidney, blood stasis block Du channel. The principle of treatment should be tonifying the liver and kidney, warming and dredging Du-channel, eliminating blood stasis and dredging the channel. The root of CSM is degeneration of cervical vertebrae according to western medicine. The pathogenis is: Degeneration of cervical intervertebral disk caused disk hemiation and narrowing of intervertebral space, then lead to instability of cervical vertebrae, hyperplasia of Lig. Flavum, ostesphyte formation, et al, which could compress the spinal cord and/or the blood vessels which supply the spinal cord with the help of the inducements such as developmental stenosis of the spine canal, inflammation, injury. In the end the corresponding clinical symptom occurs. Operation and non-operation are both methods to treat CSM Non-operative methods include limitting activity, traction, drug and blocking, et al. Operation has anterior and posterior accesses. But both methods can't ensure good effectiveness. Endothlin (ET) is a potent vasoconstrictive bioactive multipeptide mainly produced by vascular endothelial cells, it playes an important role in the etiology of many diseases. The more severely spinal cord is damaged, the more high of ET level according to modem research. It is possible for ET as a potent vasoconstrictor to take part in insufficient blood injury of spinal cord. 2. Basis of formulaProf. Deng Jinfeng deems the differential diagnosis of CSM is kidney asthenia and blood stasis, the principle of treating is to tonify kidney and move blood. In the formula, Shufuzi is the best herb to dredge Bi, it can tonify the fire of Mingmen and eliminate wind, cold, wet evils; Bajitian can eliminate wind and wetness, strengthen tendon and bone; Xianmao can warm Yan and eliminate wetness; Danshen is main herb of XueFen, it can move blood and eliminate blood stasis, make the disease heal; Shudi can moisten Yin and tonify the kidney, is the helpful herb, it can strengthen the effect of tonify-kidney and make the formula warm and not dryness.
