Objective1.To provide testing services in cooperation with CBOs for men who have sex and men(MSM)by syphilis and HIV rapid detection methods,to promote the discovery of infection among MSM,focusing on transgender;2.To master the status of HIV and syphilis and its main influencing factors among transgender;3.To understand the HIV prevention demand and transmission risk of among transgender,and to provide scientific basis for taking targeted interventions.MethodsIn January 2016 to December 2016,we used the ways of outreach,network intervention and peer education to mobilize MSM to participate in the survey in Shanghai and Tianjin cities cooperating with CBOs.The questionnaire was investigated by self-designed questionnaire,and the antibody of syphilis and HIV were detected.Data related to the questionnaire and HIV and syphilis tests were entered and verified with EpiData 3.1,and then analyzed with SPSS20.0.The main analytical methods were descriptive analysis,chi-square test and Fisher's exact probability method.The variables with statistically significant differences in univariate analysis were included in the multivariate logistic regression model for further analysis.Results1.The general situation of men who have sex with men:This study investigated a total of 1365 MSM in Shanghai and Tianjin,among which 339 were transgender and 1026 were non-transgender.They were all between 18-73 years,with an average age of(29.74 ± 8.80).The survey was conducted on the basis of the unmarried young people of Han nationality,the education level is relatively high,and 75.8%of the respondents had full-time work,the average monthly income of more than 5,000 accounted for 36.6%.In the last 3 months,95.2%of the subjects had sex with men who had had anal sex,the last time 79.5%used the condoms with sexual intercourse.Nearly 3 months,50.4%of the respondents had a stable partner,62.3%had casual partners.In the past 3 months,there were 49 people who paid for a man for sexual intercourse;there were 40 people in exchange for money by sexual behavior.Within half a year,there had been 176 people using Rush Popper,the first time they used mental substances is mainly because of the pursuit of sexual stimulation,enhancing sexual function,curiosity,fashion and so on.In the past year,51.1%of the respondents received free condoms,47.2%of the respondents received free-paid lubricants,10.6%of the respondents received free AIDS intervention and counseling,48.6%of the respondents received free STD treatment.There were 194 cases of syphilis antibody positive,the infection rate of syphilis was 14.4%,multivariate Logistic regression analysis showed that at the age of 20 to 30 years,the monthly income of more than 5000,the risk of syphilis infection was high.There were 85 cases of syphilis antibody positive,the infection rate was 6.2%,multivariate Logistic regression analysis showed that high school or secondary education and college education and above,students and people who had full-time job,received AIDS intervention and counseling services,the risk of HIV infection was high.2.The general situation of transgender:This study investigated 339 transgender.The average age was(28.82 ± 7.98),the survey was conducted on the basis of the unmarried young people of Han nationality,the level of education was relatively high,and 61.9%of the respondents had full-time work,the average monthly income of 3,000-5,000 accounted for 39.5%.The people thinking of their sexual orientation of homosexuality accounted for 85.5%;the people thinking of their psychological sex for women accounted for 56%;the people liking women clothing accounted for 58.1%.The people who thought they belonged to the "Transgender" "TG" accounted for 80.2%.In the past 3 months,there were 37 people who paid for a man for sexual intercourse,accounting for 92.5%;there were 32 people in exchange for money by sexual behavior after makeup and they were all transgender,they suffered from the phenomenon of verbal insults or attack.Within half a year,they were all transgender who used methamphetamine and zero capsules.There were 79 cases of syphilis antibody positive in transgender,the infection rate was 23.3%;and there were 24 cases of HIV antibody positive,the infection rate was 7.1%.3.The general situation of non-transgender:This study investigated 1026 non-transgender.The average age was(30.04 ± 9.04).The survey was conducted on the basis of the unmarried young people of Han nationality,the education level was relatively high,and 80.5%of the respondents had full-time work,the average monthly income of more than 5,000 accounted for 38.9%.In the last 3 months,94.0%of the subjects had sex with men who had had anal sex,the last time 80.1%used the condoms with sexual intercourse.Nearly 3 months,50.4%of the respondents had a stable partner,62.3%had casual partners.In the past 3 months,there were 34 people who paid for a man for sexual intercourse,accounting for 3.3%;there were 3 people in exchange for money by sexual behavior,accounting for 0.3%.Within half a year,there were 11 people using mental substances,accounting for 1.1%.There were 115 cases of syphilis antibody positive in non-transgender,the infection rate was 11.2%;and there were 61 cases of HIV antibody positive,the infection rate was 5.9%.Conclusions1.There are psychological barriers to the transgender,and even mental health problems.They suffer from the phenomenon of verbal insults or attack and so on.2.There is a phenomenon of psychotropic substance abuse compared to non-transgender people,especially in some new drug areas.3.The prevalence of syphilis infection in transgender is higher than that of non-transgender.
