Objective To understand the prevalence of HIV,syphilis and HSV-2among men who have sex with men(MSM)in Shenzhen,and to explore the related factors of HIV,syphilis and HSV-2 infection,to provide the basis for adjusting STD intervention strategies and formulating prevention and control measures among MSM in Shenzhen.Methods The participants were recruited from MSM-frequented venues(e.g.bars,saunas,recreational and massage centers)using the time-location sampling(TLS)method.Face to face anonymous questionnaire survey was conducted by tablet computer.Then a 5ml blood sample was obtained from subject for laboratory testing.The questionnaire data was exported through tablet computer and cleaned by Excel.All analyses were performed using SPSS 24.0.Chisquare test and analysis of variance(ANOVA)were conducted to compare demographic and behavioral characteristics in different years.The χ2 trend test was used to esti-mate the trends of HIV/syphilis/ HSV-2 seropositive rate and further evaluate the age-specific trends of HIV/syphilis/HSV-2.The binary logistic regression model were used to explore the factors associated with HIV/syphilis/HSV-2 infection and analyze the related factors of commercial sexual behavior.Results1.In 2012,2014,2016 and 2018,the HIV infection rates were 15.9%,17.5%,14.3% and 8.7% respectively,showing a significant decline trend(P = 0.003);the syphilis infection rates were 20.4%,17.5%,15.8% and 14.8% respectively,showing a significant decline trend(P = 0.025);the HSV-2 infection rates were 16.7%,14.2%,18.1% and 14.0% respectively,showing no significant decline or increase trend(P = 0.617).2.In 2012,2014,2016 and 2018,syphilis and HSV-2 infection rates showed a significant uptrend with age.Only in 2018,HIV infection rate also showed an increase trend with the increase of age.3.The results of multivariate analysis of HIV infection showed that: junior high school and below(AOR: 2.27);high school education(AOR: 1.71),monthly income ≤ 3000 yuan(AOR: 1.57);3001-5000 yuan(AOR: 1.45);2-4 male partners(AOR: 1.91)in the past six months;and ≥ 5 male partners(AOR: 2.75)were risk factors for HIV infection.The living time in Shenzhen was more than 2 years(AOR: 0.58)was protective factor for HIV infection.4.The results of multivariate analysis of syphilis infection showed that: 35-44 years old(AOR: 1.97);≥ 45 years old(AOR: 3.29),junior high school education(AOR: 1.56),monthly income ≤ 3000 yuan(AOR: 1.62),widowed/ separated/divorced(AOR: 2.09),the number of male partners ≥ 5 in the past 6 months(AOR: 1.50)were risk factors for syphilis infection.The time of living in Shenzhen < 1 year(AOR: 0.53)was protective factor for syphilis infection.5.Principal component logistic regression analysis of HSV-2 infection showed that: FAC11(X1=Ever had sex with a female;X2=Gender of first sexual partner;X3=Marital status;X4=Age group)was risk factors for HSV-2 infection(AOR: 1.53).FAC21(X5=Education level;X6=Monthly income;X7=Frequency of condom use during anal sex with men in the past 6 months)was protective factor for HSV-2 infection(AOR: 0.85).FAC44(X9=History of STDsψ)was risk factors for HSV-2 infection(AOR: 1.14).6.In 2020,the total of HSV-2 infection rate among MSM was 11.2%,of which nc MSM and MSW were 7.8% and 15.8% respectively,and the difference was statistically significant(P = 0.002).Conclusions1.The prevalence of HIV and syphilis among MSM in Shenzhen has dropped significantly,it is still at a higher level.It is necessary to continue strengthening the detection and intervention of HIV and syphilis in this population,especially for the MSM with higher age,lower education level,lower monthly income,multiple sexual partners.2.The HSV-2 infection rate of MSM in Shenzhen is fluctuating and maintains a high level.It is suggested that HSV-2 can be used as the STD detection index of this population.3.The infection rate of HSV-2 among MSW is significantly higher than nc MSM,and the detection and intervention of HSV-2 should be carried out in MSW.
