| ObjectiveTo design a KAP questionnaire of intra-abdominal pressure(IAP)measurement for ICU nurses,and test reliability and validity of the scale,provide tools for KAP investigation of IAP measurement on ICU nurses.To investigate IAP measurement knowledge,attitude and behavior status of ICU nurses among tertiary-A hospitals in Jinan,Shandong Province,and analyze main factors influencing IAP measurement of ICU nurses and current methods that ICU nurses use to measure IAP,to make training suggestions according to the real situation,and provide a scientific basis for ICU nurses on-the-job training decisions.Methods1.With the KAP theory working as the theoretical framework,literature review,widely research on IAP measurement progress,guide,and simple scales at home and abroad was conducted to draw up the items pool.Through expert enquiry and semi-structured interviews,the preliminary questionnaire formed.2.The preliminary survey was processed among 165 ICU nurses by questionnaire survey using convenient sampling in tertiary-A hospitals in Jinan,Shandong Province.SPSS 20.0 package was applied for data analysis.Determination method and Pearson correlation analysis were used to filter entries.The questionnaire was tested in reliability with Cronbach's ? coefficient,binary reliability and validity with content validity,structure validity.3.With a method of multistage sampling,4 tertiary-A hospitals in Jinan,Shandong Province were chosen,and self-designed KAP forms were distributed to 305 ICU nurses satisfying criterions who work in these hospitals from May,2016 to July,2016,which reflected knowledge,attitude and behavior status of ICU nurses.And the risk factors were also analyzed.4.After collection of questionnaire,Epidatd3.1 package was applied to build a database,SPSS20.0 package was applied for statistical analysis.Statistical methods included descriptive statistics,t test,univeriate analysis,Pearson correlation analysis and multiple linear regression analysis.Results1.After twice enquiry of professional expert,project analysis and reliability&validity test,the final questionnaire was formed,which is composed of 5 dimensions,24 items,and names:The KAP questionnaire of IAP measurement for ICU nurses intertiary-A hospitals in Jinan,Shandong Province.Two point scoring method was used in the knowledge part,1 point and 0 indicating right answer and wrong,respectively.The attitude part adopts likert5 scoring method,1?5 point respect totally disagree,fairly disagree,uncertain,fairly agree,totally agree.The behavior part adopts likert5 scoring method,1?5 point respect never,occasionally,sometimes,often,always,respectively.2.Test of reliability and validity of questionnaire:the result of the exploring factors analysis showed that KMO was 0.838.The value of ?2 in the Bartlett's test was 1288.406.Five factors were obtained and the accumulating contributing rate was 52.5%,which suggested that the validity of the questionnaire was satisfying.The total Cronbach's ? coefficient of this questionnaire is 0.869,the binary reliability is 0.784.Cronbach's ? coefficient of knowledge,attitude and behavior part is 0.612,0.749,0.848.Content validity index I-CVI is 0.93?1,average content validity index S-CVI/Ave is 0.98.These results suggested that the reliability and validity of questionnaire are satisfying and it could be used for assessing the KAP of IAP measurement of the ICU nurses.3.The total score of IAP measurement knowledge was 9,average score was 7.03 ±1.41.Total score of attitude was 25,average score was 21.00 ± 2.58.Total score of behavior was 45?average score was 38.16 ± 5.41.4.The univariate analysis showed that knowledge and attitude scores of different departments were significantly different(P<0.05),while difference of behavior scores was not statistically significant(P>0.05).Attitude scores of different length of service were significantly different(P<0.05),while differences of knowledge and behavior scores were not statistically significant(P>0.05).Knowledge,attitude and behavior scores of different first degree were all significantly different(P<0.05).Knowledge scores of different highest degree were significantly different(P<0.05),while difference of behavior and attitude scores were not statistically significant(P>0.05).Attitude and behavior scores of different title were significantly different(P<0.05),while difference of knowledge scores was not statistically significant(P>0.05).5.Multiple factors analysis showed that knowledge and attitude scores of different departments were significantly different(P<0.05).And attitude scores of different ages and first degree were significantly different(P<0.05).6.Pearson correlation analysis showed that there was a positive correlation between knowledge,attitude,and behavior,the correlation coefficient were 0.35,0.37,0.38,respectively.The differences were statistically significant(P<0.05).Conclusion1.The survey reflects the involved nurses are lack of IAP measurement knowledge,especially the theory knowledge,which indicated a shortage of nursing staff with professional IAP measurement background.Thus,more training and lectures of ICU nurses in IAP measurement are urgently needed.Behavior and attitude of ICU nursed are also need to be further improved.2.Knowledge and behavior of different department ICU nurses are significantly different,which are best in surgical ICU nurses,behavior shows no difference.Thus ICU nurses of different departments should strengthen the communication and exchanges,as to the clinical practice,departments rich in experience should give lectures to other departments.3.ICU nurses with bachelor's degree or higher degree as the first degree can anticipate changes of the patient conditions and take measures timely because of solid theoretical foundation and more knowledge of disease diagnosis and treatment.To perfect the knowledge system,improve the education level,understand diseases more comprehensively,and improve nursing quality,managers of nurses should encourage on-the-job nurses to take active part in education and training.Behavior relates to age and education background,behavior score declines with the growth of age,while it rises when nurses have higher degree.4.Our research shows a positive correlation between knowledge,attitude and behavior,it means that the more knowledge ICU nurses have,the better attitude and behavior they perform in daily work.Similarly,if ICU nurses attach great importance to attitude,knowledge and behavior will be better.5.Current mainly study source of IAP measurement knowledge for ICU nurses is clinical practice and the accumulation of experience,their knowledge is lack of systematicness and comprehensiveness.However,ICU nurses hope to gain knowledge through academic conference seminars and experts lectures.Therefore,to improve knowledge and confidence of ICU nurses on IAP measurement and provide high quality care,the nursing management departments of hospital should appropriately increase the amount of specialized training,the cross training of theory and practice. |
