ObjectiveChronic obstructive pulmonary disease(COPD)is characterized by persistent airflow limitation.Even stable patients also tend to have a certain degree of difficulty in breathing.Dyspnea leads to reduced physical activity,resulting in physiological muscle atrophy,psychological depression,and social isolation in social interaction.These factors are related to each other,form a vicious spiral.So the assessment on prognosis and quality of life need to consider the physiological,psychological and social factors,in order to be more accurate.Self testing health is subjective evaluation of their health status and expectations.In monitoring the risk of death and life quality in the elderly population,the reliability of self-rated health is higher than that of objective health.Worldwide,self-rated health has been widely used in the direction of treatment,the evaluation of clinical efficacy,the formulation of health policies and the implementation of health education.By improving COPD patients with stable physiological,psychological and social aspects of health self testing,can effectively improve the prognosis and quality of life of patients.Alleviate the patient's physical pain,while providing patients with better psychological experience and social interaction experience.Most patients with stable COPD are at home to recuperate.Some literatures have pointed out that the doctor's door-to-door treatment and health education can improve the health experience of these patients to some extent,and improve their prognosis and quality of life.But at the present stage,the population of our country is large,which leads to the relative shortage of medical resources.So Many patients with stable COPD do not receive timely help.Telemedicine can ignore the limitations of time and space,making limited medical resources to greater extent.It can also save patients' valuable time,avoiding the patient's long-distance travel.To study the influence of telemedicine on self-rated health in patients with stable COPD is of great significance for improving the prognosis and quality of life in patients with COPD.MethodThe subjects of this study were COPD stable patients who were recuperating at home,and the study was done by the contrast test before and after.The telemedicine technologies in this study include electronic medical records based on family wards,remote monitoring,remote ward rounds,remote consultation,distance education and consultation,remote communication,etc..The study adopt the self rated health assessment scale(SRHMS),which refered to the definition of health from WHO and considered the situation of our country.The author is Xu Jun,et al.The scale has been proved to have good reliability,validity and responsiveness in assessing the self-rated health of all types of people.For the patients with stable COPD in this study,the scale can describe its self-rated health from three aspects of physiology,psychology and society.It can reflect the prognosis and the quality of life more accurately.Identify patients who participated in the study.The general data and self-rated health scores of all patients were counted before the intervention,and all patients were randomly divided into the remote intervention group and the control group to ensure the comparability between the two groups.The remote intervention group received telemedicine intervention,and the control group received routine visits only.After remote intervention,the self-rated health scores of two groups of COPD patients were counted again.The changes in self-rated health of the patients in the remote intervention group were analyzed and compared,and other factors were excluded.ResultA total of 20 eligible patients with stable COPD participated in the study.They were randomly divided into two groups,10 patients in each group.The general data of the two groups were not statistically different.(1)At the beginning of the study,there is no statistical difference between the two groups in their self health scores(P>0.05).(2)For the control group,there is no statistical difference before and after the study in their health scores(P>0.05).(3)For the experimental group,their health scores have statistical differences before and after the study(P<0.05).There is no statistical difference before and after the study in their physical health scores(P>0.05),and statistical differences appear in their mental health scores(P<0.05)and social health scores(P<0.05).There was no significant difference in role activity and social adaptation dimension(P>0.05).(4)After the study,there are statistical differences between the two groups in their self health scores(P<0.05).There is no statistical difference between the two groups in their physical health scores(P>0.05),and statistical differences appear in their mental health scores(P<0.05)and social health scores(P<0.05).There was no significant difference in role activity and social adaptation dimension(P>0.05).Conclusion1.Telemedicine can significantly improve self-rated health in COPD stable patients.2.In the aspect of mental health,telemedicine improves the self-rated health of patients with stable COPD.3.In the aspect of social health,telemedicine improves the self-rated health of the COPD stable patients,and the self-rated health promotion of the role activities and social adaptation dimensions is not obvious.4.In physiological health,the self-rated health improvement of telemedicine patients in COPD stable phase is not obvious.
