ObjectiveTo observe the distribution of procalcitonin(PTC)and C-reactive protein(CRP)in patients with acute urticari.To research the value of PTC and CRP for acute urticaria patients.Methods:The acute urticaria patients were prospectively collected in dermatology department in a tertiary hospital from Novenber 1st 2015 to March 30th 2016.According to the guideline,we divided the patients into mild group,moderate group and severe group.The clinical data were collected,including demographic information(gender,age,etc.),temperture,serum levels of CRP,WBC,PCT and the rash severity score.Results:1.120 patients were enrolled in the study.Of these patients,40 patients were diagnosed as mild patients,31 were moderate and 49 were severe.Among different severity patients,the first day WBC counts have no significant difference.However,the CRP and PTC values were both elevated with the increase of rash severity.2.For mild patients,there was no significant correlation between the first day WBC count,CRP and PCT value(all P>0.05).For moderate acute urticaria patients,there was mederate correlation between CRP and PCT value(r=0.691,p<0.001).For sever acute urticaria patients,there was mederate correlation between CRP and PCT value(r=0.739,p<0.001),and also between WBC and PCT value(r=0.802,p<0.001).There was mild correlation between CRP and WBC(r=0.493,p<0.001).3.Of the 120 patients,55 patients used antibiotic.With severe patients,there was no significant difference between the used antibiotic group and non-used group,for efficiency or the decline of WBC,CRP and PCT.However,compared to the non-used group,the rash faded quickly for the used group.With mild and moderate patients,there was no significant difference between the two groups,for the decline of inflammatory markers or rash score.4.Of the enrolled 120 patients,41 patients had definite infection while the left 79 patients had undefined infection.Age and female proportion were significantly higher in the infection group.The level of serum WBC,CRP,PCT and rash score were similar between the two groups.5.Acorrding to the ROC curve analysis,the serum PCT and CRP level of the first day could well distinguish the severity of rash,the AUCs were 0.79 and 0.815 and no significance was found between PCT and CRP,the differ of AUCs was 0.023,95%CI was-0.055?0.101,P=0.564.However,the distinguish ability of serum PCT was superior than WBC,the differ of AUCs was 0.256,95%CI was 0.137?0.376,P<0.01.The distinguish ability of serum CRP was superior than WBC,the differ of AUCs was 0.279,95%CI was 0.155?0.404,P<0.01.Conclusion:Firstly,the WBC count and CRP value were elevated in acute urticaria patients on early stage,and PCT value stayed in normal range.Secondly,there was positive correlation between the PTC and CRP values and the severity of rash.It was helpful to determine whether for severe urticaria patients.However,it had no significant value for PCT and CRP to determine whether co-infection,at least for mild and moderate patients.
