Objective:To investigate the effect of procalcitonin levels on the prognosis of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)and the relationship between procalcitonin levels and other infectious indicators in patients with chronic obstructive pulmonary disease(COPD).Methods:Inpatients diagnosed as"acute exacerbation of chronic obstructive pulmonary disease"admitted to Department of Respiratory and Critical Care Medicine of the First Affiliated Hospital of Guangxi Medical University from January 2017 to June 2018 were selected as subjects.All hospitalized patients were grouped by inclusion and exclusion criteria.The patients with PCT?0.1 ng/ml were positive and patients with PCT<0.1 ng/ml were negative.The selected patients'serum C-reactive protein,erythrocyte sedimentation rate,white blood cells,neutrophil,hospitalization days and hospitalization costs were collected.At the same time,the selected patients were followed up for half one year to observe the frequency of AECOPD recurrence and the number of deaths due to AECOPD.Statistical analysis was performed using the data collected by SPSS 23.0 software,and the result was statistically significant with P<0.05.Results:From January 2017 to June 2018,there were 792 inpatients diagnosed with"acute exacerbation of chronic obstructive pulmonary disease"admitted to Department of Respiratory and Critical Care Medicine of the First Affiliated Hospital of Guangxi Medical University.After tested in the inclusion and exclusion criteria,301 patients met the criteria(if the patient had multiple hospitalizations,the first hospitalization was considered in this study),among whom 55 patients were found with PCT?0.1 ng/ml and the other 246 patients were found with PCT<0.1 Ng/ml.According to the level of PCT,the 55 patients with PCT?0.1ng/ml were taken as the PCT positive group,and another 55patients were randomly selected from the 246 patients with PCT<0.1ng/ml(The246 patients were numbered and a random number generator were used to extract 55 numbers from them.)as the PCT negative group.Among the 55patients in the PCT positive group collected by the above methods,51 were male and 4 were female,and the average age fluctuated between 72.49±10.57.Among the 55 patients in the PCT negative group,there were 52 males and 3 females,and the average age fluctuation between 71.74±9.29.Serum CRP,ESR,WBC,NE%and hospitalization costs of patients in the PCT positive group were respectively compared with those in the PCT negative group by t test and the results were statistically significant with P<0.05.The above indicators in the PCT positive group were all higher than those in the PCT negative group.The Pearson correlation analysis was performed on PCT with CRP,ESR,WBC,NE%,and hospitalization costs of the110 patients and the Spearman analysis was performed on PCT with hospitalization days and it indicated that PCT was positively correlated with CRP,WBC,NE%,hospitalization days and hospitalization costs,and there was no significant correlation between PCT and ESR.The risk of AECOPD events within half one year in the PCT-positive group and that in the PCT-negative group were X~2 tested and the result was statistically significant with X~2=5.923 and P<0.05.The PCT and half-year recurrence frequency of the 110 patients were analyzed through Spearman correlation,and it indicated that there was no significant correlation between PCT and recurrence frequency.Conclusion:1?The PCT level of AECOPD patients was positively correlated with CRP,WBC,and NE%,and had no significant correlation with ESR.2?The level of PCT in patients with AECOPD was positively correlated with the hospitalization costs and days.Therefore,serum PCT levels in AECOPD patients can offer a reference for the required length and cost of hospitalization.3?If the PCT is 0.1 ng/ml,when the AECOPD event occurs,the probability of recurrence of AECOPD events in the group with PCT?0.1 ng/ml over the next 6 months will be significantly higher than that in the group with PCT<0.1 ng/ml,and the mortality rate is higher.
