BackgroundSeveral national and regional epidemiological surveys in China have reported increase in the prevalence of alcohol use disorders since 1980 s.Rational psychiatric intervention may reduce the harm of alcohol use disorders significantly.In order to provide scientific basis for clinical intervention,we investigated on the clinical status of psychiatric patients with alcohol use disorders in a hospital.ObjectiveTo investigate the clinical charicteristics of psychiatric patients with alchol use disorders and analysis relative factors.Method(1)AUDTI was used for screening patients with alchol use disorders among psychiatric patients from The Fifth Hospital of Luoyang City over thtee-month period from10.01.2014 to 02.10.2015.(2)Case report form,DSM-IV and BDI were used to assess the information of demographic and clinical data of the patients with alchol use disorders.Relative variables were gender,age,marital status,employment status,income levels,AUDIT scores,(in a four-weeks window)the average frequency of weekly alcohol consumption and the average number of excessive drinking days,deseases(physical illness,mental disorder due to alcohol,other mental disorders),use of therapeutic drugs(for physical illness,mental disorder due to alchohol and other mental disorder),Beck scores,alcohol dependence,hospitalization.(3)The patients with alchol use disorders were divided into alchol dependent group and non-alchol dependent group according to DSM-IV.The depression level was divided into four degress: non-depression,slight depression,normal depression,and severe depression according to scores of BDI.The chi-square test was used to assess association for alcohol dependence and depression.(4)We estimated the proportion of inpatients in the patients with alchol use disorders.Chi-square test and Logistic Regression analysis were used to analysis the influence factors of hospitalization of the patients with alchol use disorders.Result(1)2056 psychiatric patients completed the screening and respond rate was 99.85%,the detection rate of alcoho1 use disorderors in psychiatric department was 9.35%,and that in psychiatric clinlic was 8.53%,that in psychiatric wards was10.84%.(2)The study population consisted of 193 patients with mean age of 35.15 ±11.05(years old),the proportion of males was 91.19%,non-employed 40.93%,nonmarried/cohabiting 41.97%,low-incomer(monthly per capita household income <? 3000)50.26%.(3)In a four-weeks window,there were 23.83%,28.50%,47.67% of the patients in favor of strong white wine,low-alcohol white wine,beer or other wine respectively;the average frequency of weekly alcohol consumption was 3.97±2.51 and the average number of excessive drinking days was 10.96±10.57.(4)The proportion of patients with AUDIT?13/15 was 45.08%,and that of patients with alcohol dependencts 28.05%.Physical illness,mental disorder due to alcohol,other mental disorders were the three main diseases,accounting for 40.93,55.96 and 70.47%,respectively.The use rate of therapeutic drugs for physical illness was 31.61%,for mental disorder due to alchohol 15.54%,for other mental disorder was 66.32%.(5)Alcohol dependence was associated with depression.Cramer's V coefficient was 0.53,Pearson contimgency coefficient 0.47.Alignment contimgency coefficient r'= 0.66.There was a close association between the two variables.(6)The hospitalization rate of the study population was 40.93%.To analysis single factors of hospitalization rates,we found that the impact of gender,age,AUDIT scores,employment status,income levels,marital status,alcohol dependence,(in a four-weeks window)the average frequency of weekly alcohol consumption and the average number of excessive drinking days were statistically significant.The result of Multivariate Logistic regression analysis was: the hospitalization of the patients with alchol use disorders was affected mainly by age,alcohol dependence and monthly per capita household income.Conclusion(1)In psychiatric department,the detection rate for alcoho1 use disorderors was high,patients with alcohol use disorders was with complicated condition and severe alcohol dependence,the use rates of therapeutic drugs were low.Measures need to be taken as early as possible.(2)As to patients with alcohol use disorders in psychiatric department,their manner of seeking medical help was likely affected by age,alcohol dependence and monthly per capita household income.
